Job summary

Are you looking for the opportunity to work in an NHS-Funded Care Team (NHS Continuing Healthcare, Acquired Brain Injury, Discharge-to-Assess)? Do you have the skills, values and knowledge to assess, care plan and deliver person-centred care for vulnerable adults? Your role is key to ensuring that we commissioning effective and appropriate care packages for our population. We are looking for an experienced nurse assessor to manage a caseload of patients across the community in an integrated and multi-agency approach. You will be responsible for the day to day management of a case load, working autonomously and as an integral member of the team.

We are looking for nurses with a good knowledge and experience in CHC/ funded health care to join our Southend/CP&R team and we will work with you to further develop your skills and knowledge as part of the Nursing and Quality Directorate. The South East Funded care team is a happy dynamic team, covering the Southend and Castle point and Rochford area.

Main duties of the job

The post holder will work as a nurse member of NHS Funded Care Service within the Nursing and Quality Directorate and will be responsible (under the guidance and support of the CHC Senior Assessor/Case Manager) to proactively manage caseloads of people receiving NHS Funded Care; such as Acquired Brain Injury or NHS Continuing Healthcare.

They will also implement the National Service Framework for NHS Continuing Healthcare (2012), NHS Funded Nursing Care and Personal Health Budgets (PHBs) for residents within the responsibility of Southend or Castle Point & Rochford Clinical Commissioning Group.

CLINICAL RESPONSIBILITIES

• Continuing responsibility for a caseload as directed by the CHC Clinical Lead or Senior CHC Assessor.

• To ensure identified priorities are addressed and improvement outcomes are demonstrated for care providers.

• To undertake holistic assessment in conjunction with the multi-disciplinary team for patients identified as requiring CHC assessment. This will include liaising with other professionals, carers and family members to identify care needs and plan to meet these needs.

• To maximise the potential of anyone prior to receiving a CHC assessment, by accessing appropriate alternative commissioned services.

• To identify the risks presented by the needs of individuals and develop person-centered, cost effective care plans, working with specialist health professionals to manage these risks and individual’s needs.

• To guarantee providers are evidencing that packages of care are based on patient needs to ensure value for money.

• To monitor and review care packages with other professionals involved in the care provision, undertaking scheduled and unscheduled reviews for patients with CHC needs, ensuring that the care provision, quality of care and the setting is appropriate to need.

• To co-ordinate and ensure the timely, effective and safe discharge/transfer of those patients who no longer require on-going NHS Funded Care or meet the NHS Continuing Healthcare criteria.

• To develop and evaluate delivery of care through continuous review of evidence based clinical standards and ensure these are within agreed framework incorporating policies, protocols, procedures and Standards for Better Health.

• To support, facilitate and take an active approach to on-going professional development.

• To support and work with carers and wider family members in an empathic and professional manner.

• To manage patient identifiable information in line with CCG policies and Caldicott principles.

• To identify and manage issues in line with local Safeguarding, DoLS/DoL and ‘Best Interest’ procedures (Mental Capacity Act 2005).

• To ensure care is delivered in line with the Mental Capacity Act 2005.

• To manage care at the end of life linking with palliative care and end of life care teams to provide this.

• To liaise with specialist professionals (eg, Community Matrons, District Nursing, specialist nurses, and consultants) in the management of complex health issues for patients meeting the continuing healthcare criteria.
