Main duties of the job
As a Continuing Healthcare Nurse Assessor, you will work under the guidance and support of the senior clinical team leads to implement the National Service Framework for NHS Continuing Healthcare and NHS Funded Nursing Care for residents in Mid Essex. You will promote best practice and provide professional advice to nursing homes and other providers, promote good relationships and be able to liaise with colleagues from other disciplines both internally and externally to the Clinical Commissioning Group.
You will have excellent communication and assessment skills. Be able to show a robust approach to problem solving and the ability to prioritise your own workload. Integrated
Care Systems (ICSs) are partnerships of health and care organisations that come
together to plan and deliver joined up services and to improve the health of people

1stst July 2022 CCG’s will cease to exist and the Mid and South
Integrated Care Board (ICB) will be established. If you are employed by the CCG

ROLE DESCRIPTION / SUMMARY
The CHC Nurse Assessor will work under the guidance and support of the CHC Clinical Lead Nurse and CHC Operational Lead Nurse to implement the National Service Framework for NHS Continuing Healthcare and NHS funded Nursing Care for residents in Mid Essex. He/she will promote best practice and provide professional advice to nursing homes and other providers, promoting good relationships and liaising with colleagues from other disciplines both internally and externally to the CCG.
KEY RESPONSIBILITIES
- To work with provider services to assist in the development of competent assessors to undertake assessments and reviews.
- Responsible for collating evidence and information for the Decision Support Tool and liaising with the multidisciplinary team in order for a team recommendation to be made for eligibility for Continuing Healthcare funding.
- Present information for panel deliberation.
- Manage day to day communication with other team members within the continuing healthcare team.
- Responsible for recording client contacts and communications on Broadcare.
- To keep the CHC Clinical Lead Nurse and CHC Operational Lead Nurse informed of workload and resources required.
- To work within the National Framework for Continuing Healthcare and NHS-Funded Nursing care.
- Exhibit a sense of responsibility and accountability for the allocated caseload and coordinate care in all settings in the patient pathway.
- Act in an educational and advisory role to primary care staff and other partner organisations in order to share knowledge concerning Continuing Healthcare and NHS-funded Nursing Care.
- Apply any relevant local policies to assessments and CHC activity.
Dependent on allocated case load the role may include:
Clinical and Professional Duties
- The post holder will undertake statutory reviews and ensure that client’s needs have been met and progressed through the appropriate care pathway within agreed time frames. To ensure all reviews, reassessments include appropriate reports and evidence to support the level of care need identified within the Health Needs Checklist / Decision Support Tool / Needs Portrayal process.
- To provide contemporaneous records on all clients and reports as necessary adhering to NMC standards for record keeping.
- To coordinate and facilitate ‘fast track’ patients in line with operational policy and within agreed timescales. Ensuring the patients that are eligible receives a package of care or placement appropriate to their needs ensuring best value.
Communication Skills
- To assess, review and plan an appropriate care pathway, in liaison with other healthcare professionals involved with the client.
- As part of the review process, ensure a review of placement, care package is also undertaken to ensure that all care needs are being met and to ensure best value for the funding authority. When appropriate proceed to the NHS- funded nursing care process.
- Ensure patient and family are informed throughout the process and aware of set timeframes.
- Ensure effective and timely communication regarding the review process and the outcome of the review, with all professionals involved and with the client and their representatives.
Analytical and Judgement Skills
- Clinical assessment skills required, nursing needs, holistic approach to the totality of the client’ needs.
- To make complex operational judgements and attempt to resolve conflicting views/ reconcile inter/intra professional differences of opinion including crisis intervention.
- To follow safeguarding policies and guidelines, and know how to seek specialist advice.
- To work in an integrated approach with partner agencies, to support the provision of assurance that CCG commissioned services, are safeguarding adults at risk.
- To be able to recognise the indicators of abuse and how to act on them.
- To be alert to the possibility of harm to children, young people and adults at risk through abuse and neglect. This includes being aware of adults who may find parenting difficult.
Patient/Client Care
- To maintain effective communication on issues in complex situations arising from the application of NHS Continuing Care criteria and the outcomes, including a range of professionals including GP’s and Health Professionals.
- To communicate decisions to clients and families regarding funding decisions which include distressing information whereby health funding may not be approved or has been reduced or withdrawn, or a decision has been deferred for further review.
- When appropriate assess clients for funded nursing care, ensuring timely reviews, and to engage with clients and their representatives to explain the assessment/review process.
- Support the CHC Clinical Lead Nurse in ensuring patient experience is captured and commitment to improving of patient experience.
Policy and Service Development
- To develop a good working knowledge of current key issues for the directorate.
- To adhere to local policies and processes.
- Flexibility to travel to meetings and other locations in order to fulfil the requirements of the post.
Planning and Organisation
- To liaise in partnership with key stakeholders in the independent sector and social care to enhance patient assessment outcomes.
- The post holder will ensure the necessary precautions are adhered to when information is transferred only disclosing patient identifiable information in line with Caldicott and Data Protection Act.
- Effectively communicate with other professionals and with voluntary organisations.
- In the absence of the CHC Nurse Manager to ensure, the team runs smoothly and any problems are escalated to the appropriate manager.
- To provide clinical support on specific pieces of work and projects as required by the dept. /directorate.
