An exciting opportunity has arisen for a Clinical Manager who is registered Allied Health Professional /Nurse and has the ability to take the lead with co-ordinating neuro navigation pathways for residents of Mid and South Essex Integrated Care Sysytem. alongside supporting a small team and its development.

The Neuro Rehabilitation team helps people with neurological conditions who have complex needs and require rehabilitation. The team support service users affected by facilitating transitions between hospital, rehabilitation and home presenting options for local and specialist rehab, helping service users to decide best choice of care in conjunction with , who provides specialist clinical case management throughout the journey: from the acute setting, through rehab and back into the community.

Working from Home plus any travel required to maintain service delivery.

Main duties of the job

You will provide professional leadership, advice and expertise to ensure delivery of a high-quality standard of clinical assessment, review and management requiring the support of the Neuro Rehab team.

You will be expected to manage the Neuro Rehab team and work collaboratively with key partners to achieve best outcomes for affected service users across the whole of Essex.

You will provide advice, support, supervision and line-management to all members of the Neuro Rehab team, ensuring that they have the support that they require to deliver the highest quality and most appropriate services whilst driving innovation and continuous service improvement.

The role of the Mid and South Essex Integrated Care Board (MSE ICB) is to join up health and care services, improve peoples health and wellbeing and reduce health inequalities for our 1.2m population through our wider system and four Alliances. MSE ICB are committed to place clinical and professional leadership and the voice of our residents at the heart of realising all its ambitions.

This role sits within the Nursing and Quality Directorate and supports the clinical and operational lead role in the All Age Continuing Care (AACC) service, which incorporates the adult Continuing Healthcare (CHC) and Children, Young Peoples Continuing Care (CYPCC) teams and the Neuro Navigation team.

The AACC service delivers the statutory duties of the ICB to assess and to provide for services to meet the identified needs of those found eligible for Continuing Healthcare. The service delivers that duty through the implementation of the National Framework for Continuing Healthcare and NHS Funded Nursing Care (2018) and the National Framework for Children and Young Peoples Continuing Care (2016), working in accordance with the AACC Operational Policy.

The Neuro Rehabilitation team helps people with neurological conditions who have complex needs and require rehabilitation. The team support service users affected by

  • facilitating transitions between hospital, rehab and home
  • presenting options for local and specialist rehab, helping service user to decide best choice
  • delivered by a clinical navigator, who provides specialist clinical case management throughout the journey: from the acute setting, through rehab and back into the community

COMPETENCY BASED KEY DELIVERABLES

  • To provide clinical leadership and management of the Neuro Rehab team and wider AACC team as appropriate
  • They will be expected to support, motivate and develop staff within the team to ensure that they are able to deliver both organisational and national objectives
  • To forge close and positive working relationships with internal and external stakeholders, in order to achieve the delivery of a safe and effective Neuro Rehab service
  • To utilise expert Neuro Rehab knowledge and experience to stakeholders such as colleagues within the ICB, Acute Trust, Mental Health Trust, GPs, Social Care, Independent Providers, Nursing and Residential Home staff regarding the provision of highly specialist care support requirements for this specific group of service users
  • To ensure (and in the most complex cases, provide) assessment, care and support planning, ongoing case management and review for all relevant services users
  • To undertake or coordinate appropriate assessments, approvals and notifications of deprivations of liberty, maintaining comprehensive records
  • To ensure the agreed care and support package continues to meet the individuals assessed health and associated care/support needs and their agreed outcomes
  • Responsible for the recruitment of clinical staff within the team, acts as chair of recruitment panels and is the responsible recruiting officer
  • To support, motivate and develop staff within the team to ensure that they are able to deliver both organisational and national objectives
