within our service and are looking for a highly motivated and enthusiastic
professional to join our supportive and dynamic team.
To lead and oversee the
day-to-day management and co-ordination of the Continuing Healthcare Learning
Disability workstream within the Complex Cases Team and the process for
undertaking NHS Continuing Healthcare Assessments, applying the Department of
Healths National Framework for NHS Continuing Healthcare and NHS-funded
Nursing Care.
To provide specialist advice
and undertake clinical assessments as required with respect to the process for
NHS CHC Assessment/Review for NHS Continuing Healthcare, to clients, health and
social care professionals and other agencies, within the relevant and specific
team, with the ability for cross cover across the directorate as and when
required.
Main duties of the job
We are looking for a
registered professional who is experienced in working with people who have Learning
Disabilities, complex physical and mental health needs.
NHS Continuing Healthcare
(CHC) is the name given to a package of care that is arranged and funded solely
by the NHS for individuals who are not in hospital, but who have complex
ongoing healthcare needs.
The Complex Cases Team is
responsible for the decision making and commissioning processes, working
closely with the Local Authorities throughout Cambridgeshire &Peterborough.
Successful candidates will
have excellent communication and assessment skills; be resilient, able to work
autonomously but also work as part of a team. This is a home-based role, and
post holders will be expected to be able to travel across Cambridgeshire and
Peterborough.
You will be joining a
fast-paced team that employs the use of holistic nursing, inquisitive thinking
and a focus on high quality. The nature of the role is autonomous and
challenging but with the benefit of supportive colleagues.
