Job summary
We require a Learning Disabilities Nurse or someone
with similar experience you will lead a team of reviewers implementing the
learning Disabilities Mortality review (LeDeR) programme of work and ensuring
recommendations around health inequalities for people with a learning
disability and autistic people are translated into delivery of quality
improvement across the integrated care system.
You will become part of a
wider team supporting commissioning and service transformation for people with
a learning disability and autistic people, working mostly from home, you will receive regular
individual and group support, on-going CPD and training, and support within
this role to help further develop your skills and competencies in this
specialist field
Main duties of the job
The post
holder will oversee the LeDeR Team (within the Nursing and Quality Directorate)
& will ensure the delivery of the LeDeR programme which requires conducting
retrospective reviews of each death of individuals with a learning disability and autistic people that have been notified to the Cambridgeshire & Peterborough LeDeR programme.
The post holder will be expected
to effectively liaise and collaborate with all key partners and stakeholders at
all levels. This will involve working and liaising closely with a range of
colleagues from the statutory, non-statutory and independent/voluntary sectors,
the spectrum of health and social care services across Cambridge and
Peterborough ICS , Local Authority partners, Primary Care, other NHS Provider
trusts (acute, mental health and community) and other relevant statutory and 3rd sector organisations, who are commissioned to provide services as well as
patient and carer groups and families of those who have died, to support the
local programme.
The role has a particular focus on the Local Area
Contact functions, developing and maintaining the whole programme in line with
the best national practice, and ensuring that learning recommendations to
address health inequalities are shared and implemented across the Integrated
care System (ICS).

Work internally and externally to deliver the responsibilities of the

Develop resources to support Reviewers to conduct
quality reviews and enable Reviewers to adequately support bereaved carers and
families with whom they come into contact through the course of undertaking a
review.
Work with others to ensure effective processes are
put into place to streamline the completion of investigations/reviews where
processes have the potential to overlap (for example Child Death Overview
panel, Safeguarding Adults Review, Coroners Inquest)
Ensure that Safeguarding principles are considered
and applied within all elements of the LeDeR programme and establish links with
key local Safeguarding contacts and ensure that any concerns are reported as
per Safeguarding Children and Adults policies.
With support from the LeDeR
Steering Groups develop a communication and briefing approach in sharing
lessons learned from mortality reviews both locally and nationally for
dissemination across the health and care
Support the identification and sharing of best
practice in the LeDeR process.
Share, present and submit the
learning from completed reviews with the local teams and other partners to
support identification of learning that can be actioned. To collate and analyse
information from completed reviews and feedback to local groups and other
forums as required.
Have the ability to convey and
present complex, sensitive and possibly contentious information to a range of
audiences - this information includes, but is not limited to, feedback to and
from families of deceased clients as well as learning and feedback from reviews
which may challenge system partners and local groups.
Ensure reviews and associated
outcomes are delivered on time to quality standards. Collaborate with
colleagues in Quality Assurance to review provider assurance where
recommendations from LeDeR reviews identify shortfalls in practice.
Encourage and promote a culture
that values openness, honesty, rigour, and challenge.
Maintain their knowledge and
expertise on local and national policy and demonstrate personal commitment to
continuous self-development and implementation of local service improvement
actions.
Maintain high standards of
documentation and adhere to the national LeDeR programme requirements.
