Job summary

As a Childrens Continuing Care Nurse, you will ensure that high quality assessments of needs are produced by working in partnership with the child and their family alongside health, social care and education colleagues to commission and monitor high quality care packages. You will support the best possible discharges from hospital and undertake commissioning case management for children with complex health and NHS Continuing Care needs.

You will hold a key role in the Maternity and Childrens Commissioning Team, which is part of the Nursing and Quality Directorate at Cambridgeshire and Peterborough Integrated Care Board. The team are a dynamic and friendly group of co-workers who are all passionate about the healthcare needs of babies, children, young people and their families.

Main duties of the job

The role of the Continuing Care Nurse Assessor includes:

- Organising and chairing full MDT meetings with the child/young person, their family and professionals to complete the decision support tool with professional curiosity in order to clinically assess the level of need.

- Assess patients with long term complex care requirements against the Childrens Continuing Care National Framework; including working in line with the local Continuing Care Policy for Children and Young People

- Ensure a child/young person focused approach in identifying health needs and facilitating the most appropriate care provision for individuals.

-To be involved in the ongoing development of resources and expertise within the Childrens Continuing Care Team in order to meet both National and ICB objectives.

-To promote collaborative working across Local Authority and Education, working to implement the SEND pathway.

-Work in partnership with NHS and social care colleagues.

-Support assessments of children and young people to ensure the correct provision of equipment.

-Assess risk that may affect patients or care providers

-Assess and plan for smooth transition to adult services

-Contribute to the development of integrated working with Local Authority and Education colleagues.

-Promote equality, diversity and rights

-Work within relevant legislation for example Health and safety, NMC Code of Professional conduct.

  • Ensure compliance with our clinical governance framework, you will motivate colleagues, champion service user safety and dignity and drive continuous improvement.
  • Facilitating MDT meetings and completion of assessments and annual reviews.
  • Implementation of packages of care, including PHBs (Personal Health Budgets).
  • Facilitate the undertaking of NHS Continuing Healthcare Assessments and reviews, ensuring that the care domains within the National Framework for NHS Continuing Care are considered and applied against each continuing healthcare Assessment; with detailed evidence collated and provided in each domain for each decision made.
  • Risk assessing and risk mitigation should be evidenced throughout the Continuing Care workplan.
  • To recommend if a child/young person meets eligibility Criteria for NHS Continuing Care.
  • Uphold a high standard of communication between families and professionals, ensuring the child/young person continues to be at the centre of care.
  • Complete on-going reviews of the eligibility to continue to receive NHS Continuing care funding in accordance with the National scoring framework, working in collaboration with colleagues from Social Care and Provider Services.
  • Assess and identify appropriate care packages and placements to meet the clinical needs of the child/young person.
  • Understanding of MCA and Dols.
  • Signposting to necessary services.
  • Work closely with health, education and social care professionals in the identification of future care options when a child/young person does meet the eligibility criteria for NHS Continuing Care, ensuring sensitivity, empathy, and reassurance to child/young person and their representatives.
  • Conduct training on behalf of Continuing Care relating to the National Framework, Fast Track pathway, Personal Health Budgets.
  • To promote awareness of NHS continuing Health Care to provide a clear understanding to our system partners. This may include in-reaching into hospitals, care homes and hospices.
  • Travel across Cambridgeshire and Peterborough to visit families, schools and short break provisions.
  • Ensure safeguarding of children and young people is upheld at all times.
  • Participate in quality assurance in line with standards of practice, if a concern is identified with a provider escalate through the appropriate channels.
  • To uphold the NMC code of conduct throughout practice upholding a high level of care for each child/young person.
  • Work with initiative and as part of a wider collaborative team for the safe delivery of care for each child/young person eligible for Continuing Care funding.
  • Utilise of a high level of communication to answer questions with clarity and sensitivity. The aim is to impart knowledge and promote a robust and consistent process.
