Job summary

To manage and develop services in relation to the National Framework for NHS Continuing Healthcare for Adults. Ensuring high quality, patient focused services to meet the complex needs of those eligible for NHS CHC in BSOL in line with strategic goals and national policy.

The post holder is responsible for ensuring the safe and effective management of the NHS Continuing Care Service across Birmingham and Solihull ICB and the effective and efficient management of resources of the service.

Lead the service in ensuring, that for individuals who are eligible for NHS Continuing Healthcare, and wish to receive a Personal Health Budget, the service is fully able to deliver this, with appropriate policy, procedures and governance arrangements in place.

Provide clinical and managerial leadership to the Continuing Healthcare team, to ensure a high standard of patient focused assessment, decision making and care management.

Main duties of the job

he post holder will be expected to work collaboratively with a wide range of external stakeholders to implement a local vision of Continuing Healthcare

Services, ensuring the delivery of key quality outcomes and strategic objectives.

The post holder is responsible for overseeing the commissioning of care packages for adults funded under NHS CHC.

The post holder will take the strategic lead for the commissioning Level 2 Acquired Brain Injury Pathway for individuals living in BSOL. Working in partnership with ICBs, and other key stakeholders, including providers, carers, service users and the voluntary sector, to ensure that the commissioning reflects national and local priorities, and high-quality standards of practice.

Responsible for providing senior professional expertise and advice to the operational CHC team, ICB and external stakeholders to ensure the delivery of a quality and safe CHC service.

The post holder will define current processes, facilitate discussions and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.

  • Responsible for meeting external KPI requirements for NHSE 28 days and funded nursing care.

To support the business and performance team for the submissions required to NHSE CHAT Assurance tool, Funded Nursing care reporting, and any other statutory requirements.

The post holder will be required to communicate and provide highly complex information to a wide range of internal and external stakeholders, including service users, wider public, public health, specialist commissioning, Local Authority / Education / Strategic Commissioning Team, NHS England and clinical networks to ensure a collaborative joined up approach to commissioning e.g. across the universal and targeted pathways, for those with Continuing Healthcare needs.

Nurture key relationships and maintain networks internally and externally, including national networks. Develop relationships with individual service users and service user engagement forums to ensure CHC eligible individuals are involved in coproduction and management of individual packages of care.

Implementing and monitoring the use of NHS Englands NHS Continuing Healthcare Competency Framework, to help ensure that all staff within the service are competent to deliver the requirements of their roles.

Sourcing appropriate training to meet the needs of the service

Using feedback from individuals being assessed for or receiving services,

or from their representatives, to inform training. Ensuring any learning from

incidents, case reviews and complaints is used to improve practice, and

implementing any changes to training as required.

Carrying out audits and supporting others to carry out audits, using those audits to bring about service improvement, inform training needs and /or policy refresh.

Present highly complex information about projects, initiatives and services to a wide range of stakeholders in a formal setting.
