Job summary

NHS Continuing Healthcare (CHC)/ Children's continuing care (CCC) is a package of continuing care that has been arranged and funded solely by the NHS to meet a person's physical and mental health needs arising from disability, accident or illness. It is for vulnerable people with complex, intense and unpredictable needs or people who are rapidly deteriorating and possibly entering a terminal phase.

This is a part-time position, 22.5 Hours per week.

Main duties of the job

NHS Frimley ICB's East Berkshire Continuing Healthcare Service is seeking to recruit a highly motivated, very well organised and experienced practitioner with qualifications in LD, MH or General nursing, Children nursing or an Allied professional i.e. Physiotherapist, Occupational Therapist, Social Worker to join the team of CHC/CCC assessors to support in the processing of Continuing Healthcare/ Continuing Care and Funded Nursing Care assessments, reviews and case management. You will have knowledge of the National Framework for Continuing Healthcare (July 2022)/ Continuing Care (January 2016) and be able to:

  • Coordinate multi-disciplinary meetings with a range of professionals i.e. social care, care homes, other providers etc.
  • Undertake high quality holistic assessments of patients, clearly identifying needs using assessment tools as required.
  • Manage a caseload of complex cases.
  • Cover as Duty Nurse within the Continuing Healthcare Service.

You will be a key member of the team and will need to demonstrate skills in IT, report writing, managing complex patients. You will also have excellent verbal and written communication and have the ability to work in stressful situations and be able to prioritise workloads to meet deadlines. You will be working in partnership with our local authorities and experience of multi-disciplinary working is essential. You will also be working closely with families to support the delivery of individualised care in the community for children and young people with continuing care needs.

Knowledge, Training and Experience

Essential

  • Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area
  • Further training or significant experience in project management, financial management or supporting change management processes.
  • Experience and understanding of evaluating and measuring the performance of health services.
  • Experience in communications and stakeholder management.
  • At least 3 years post registration experience of working within a multi-disciplinary team.
  • Experience of undertaking highly complex, clinical assessments.
  • Experience of working in older persons care in NHS or Independent setting.
  • Experience of working with patients with multiple or complex care needs.
  • Experience of assessing complex needs and planning and evaluating complex care packages
  • A good understanding of the health and social care environment and roles and responsibilities within it.
  • Knowledge and understanding of safeguarding of vulnerable adults.
  • In depth knowledge of NHS Continuing Healthcare legislation.
  • Knowledge of Mental Capacity Act.
