Job summary

The post holder will be a registered nurse who will work within the All Age Continuing Care Service across Cheshire delivering optimum holistic care to service users with a learning disability identified for NHS healthcare funding. The post holder will be actively involved in ensuring that service users with a learning disability meet the eligibility criteria for NHS Continuing Healthcare and are reviewed in line with national and local policies and procedures, as well as those with a learning disability eligible for Section 117.

The post holder will be an experienced practitioner with knowledge and understanding of health inequalities, a sense of advocacy and a drive to engage all partners to bring about positive change for our people across Cheshire with learning disabilities and /or Autism, working collaboratively with the wider ICB team as well as local authority and CWP.

The post holder will be part of the Learning Disability and Mental Health team that currently works across Cheshire East and Cheshire West, this role is to work in Cheshire East, based either from Macclesfield or Crewe with a hybrid approach to working from home as well.

Main duties of the job

To assess, commission, plan and procure packages of care for service users with a learning disability / s117 Aftercare in line with Cheshire's All Age Continuing Care (AACC) Commissioning Policy.

The post holder will be responsible for co-ordinating relevant assessments and completing documentation in order to commission a package of care.

The post holder must have an understanding of the Mental Capacity Act and Deprivation of Liberty Safeguards.

Work collaboratively and constructively, as part of the MH/LD complex care team, as well as with multi professionals and multi-agency organisations to ensure individuals' needs are met, particularly in relation to ongoing care needs and discharge arrangements.

Provide case management and review for service users with a learning disability working in partnership with Local Authorities.

Recording and accurately inputting information onto the electronic systems used in All Age Continuing Healthcare, adhering to NMC standards for record keeping, ensuring data systems are kept up to date and maintained in line with standard operating policy and procedures.

Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines.

To facilitate the delivery and implantation of Personal Health Budgets, ensuring the correct policy, procedure and governance arrangements are applied.

To adhere to the National Framework and all Cheshire and Merseyside ICB policies and procedures as well as implement policy and guidance in respect of Section 117 of the Mental Health Act and associated policy areas

To deputise for the clinical lead as required.

The post holder will also be asked to support work across areas of the Transforming Care Programme, supporting the delivery of Care (Education) and Treatment Reviews, chairing meetings and attending Quality Assurance reviews.

  • Educated to degree or equivalent level of qualification or experience in working at a similar level in specialist area
  • Nursing qualification with current registration
  • Evidence of post registration education and training
