Job summary
The post holder will be a registered nurse who will work within the All Age Continuing Care Service across Cheshire delivering optimum holistic care to service users with needs coming from their mental health, learning disability and/or neurodiversity who are identified for NHS healthcare funding.
The post holder will be actively involved in ensuring that service users with a learning disability and/or needs arising from their mental health meet the eligibility criteria for NHS Continuing Healthcare and are reviewed in line with national and local policies and procedures, as well as those with a learning disability eligible for Section 117.
A main part of the role will be around the Transforming Care Programme (TCP), ensuring individuals with a learning disability and/or autism are discharged from hospital in a timely and safe manner.
The post holder will be an experienced practitioner with knowledge and understanding of health inequalities, a sense of advocacy and a drive to engage all partners to bring about positive change for our people across Cheshire with needs arising from their mental health, learning disability and /or Autism, working collaboratively with the wider ICB team as well as local authority and CWP.
The post holder will be part of the Learning Disability and Mental Health team that currently works across Cheshire East and Cheshire West.
Main duties of the job
To assess, commission, plan and procure packages of care for service users with needs arising from their mental health, learning disability, and/or autism, in line with Cheshire's All Age Continuing Care (AACC) Commissioning Policy. This is for individuals eligible for continuous healthcare (CHC) funding or funding under S117 aftercare.
The post holder will be responsible for co-ordinating relevant assessments and completing documentation in order to commission a package of care.
Chairing and participating in CTR meetings as well as leading on sit and see visits for patients under TCP.
The post holder must have an understanding of the Mental Capacity Act and Deprivation of Liberty Safeguards.
We expect the successful applicant to work collaboratively and constructively, as part of the MH/LD complex care team, as well as with multi professionals and multi-agency organisations to ensure individuals' needs are met, particularly in relation to ongoing care needs and discharge arrangements.
