Job summary
The NHS in
South West London is working on a long-term plan to transform local health
services for both emergency and planned hospital care, primary and community
services, cancer, and mental health.
The South West London system has an ambitious agenda for
improving health services and is implementing one of the largest sustainability
and transformation programmes in the NHS, which includes a significant
reconfiguration of services across multiple organisations.
Our vision for health in South West London, as described in
the Sustainability and Transformation Plan (STP) is:
People live longer, healthier lives. They are
supported to look after themselves and those they care for. They have
access to high quality, joined up health and care services when they need them
that deliver better health outcomes and a lower cost of provision to the
system.
To support this vision, a single South West London Clinical
Commissioning Group (SWL ICB) has been formed to commission high quality,
effective and joined up health care for the population of the 6 South West
London boroughs (Croydon, Kingston, Merton, Richmond, Sutton and Wandsworth).
The SWL ICB will retain a strong local presence and focus within each of the
boroughs whilst working together in key areas across South West London to deliver
transformed and sustainable services.

The post holder will continue to mobilise and drive the new South West London Key Working Service. Full geographical roll out across South West London will be achieved by March 2023. The postholder must work closely with system partners ensuring the developed model works for them. This is a permanent post.
The post holder will be responsible for:
- Leading the SWL Key Working Service, in close collaboration with Place Based Transformation Leads, (the Key Worker Service will increase support provided to families at Place Based borough level)
- Core service management activities including planning, progress monitoring, risk and issue management and meeting coordination.
- Actively engaging and working collaboratively with colleagues in Place Teams, lead clinicians and partners across SWL to support service delivery, including coordinating complex activities
- Completing work and undertaking analysis as required to drive forward service delivery
- Monitoring service delivery and completing update reports for a variety of service groups and Boards
- Developing and updating processes, standard operating procedure, and documents to ensure an effective and sustainable infrastructure and model for delivery
- Line management of specialist keyworkers and key workers for the Key Working Service.

- Detailed understanding of relevant health and social care legislation
- Understanding of business and service planning processes within the NHS or local government.
