Senior Nurse Assessor

Job summary

This role will lead and manage a team of assessors who administer and undertake assessment of the patient's eligibility for Continuing Healthcare (CHC) and Funded Nursing Care (FNC) in order to plan and commission appropriate care packages to meet their needs, monitor that the care packages continue to meet their needs and review the planned care to ensure it provides a high standard of quality and safety.

Main duties of the job

The post holder will be responsible for a team delivering assessments of individual'seligibility for Continuing Healthcare and Funded Nursing Care and for reviewing carepackages for existing patients.

The post holder will ensure that assessments and reviewsare carried out in a timely manner according to national specifications.

The post holder will work with all stakeholders including patients and, where relevant, theirfamilies, to ensure that funding decisions are taken in a transparent and coherent manner.

The post holder will ensure that the assessor team keep accurate records and are able todemonstrate that funding recommendations are evidence based and made according to thecriteria described within national CHC frameworks.

The post holder will contribute to the clinical management function of the CHC service,forming part of a senior duty roster to handle fast track applications, review checklistreferrals, manage complex cases and sign off assessment ratifications.
