Job summary
We are looking for an outstanding individual to lead our work across the Merton Health and Care Partnership. The postholder will develop and maintain effective partnerships at the most senior level between statutory and non-statutory agencies involved in promoting health and well-being within the borough of Merton. This will involve oversight of the development and delivery of the Merton Health and Care Plan using a Programme approach and specifically how the local health system addresses health inequalities. The post is hosted by South West London ICB but is a joint appointment with London Borough of Merton.
Main duties of the job
Merton Health and Care Together (MHCT) is the borough based 'Place Committee' for Merton with senior representation from across the local health and care system. MHCT has oversight of the development and delivery of the borough Health and Care Plan. The postholder will lead the Programme to deliver the Merton Health and Care Plan and the work of the borough Committee (MHCT) to improve health within the borough and address health inequalities by :
Defining the MHCT programme's quality assurance and risk framework
Encouraging innovative and creative approaches to Programme management
Ensuring there is allocation of common resources and skills within the programme's workstreams
Creating and embedding an innovative delivery framework across the MHCT collaborative programme
Developing, supporting and coaching people to use the agreed delivery framework
Maintaining an overarching Programme Plan
Constantly monitoring to ensure that the 'right' projects are being undertaken to achieve the Vision, strategic objectives and benefits of the MHCT partners.
Driving benefits realisation - and reporting progress including providing highlight and other key reports for the MHCT Board
Identifying and managing key interdependencies across the programme portfolio
Managing and implementing a Communications Plan in collaboration with whole system partners and promulgating a communication and engagement framework.

NHS South West London Integrated Care Board (ICB), as part of South West London Integrated Care System (ICS), is a partnership of organisations that come together to plan and deliver joined up health and care services to improve the lives of people in our six boroughs: Croydon, Merton, Kingston, Richmond, Sutton and Wandsworth.

- enhance productivity and value for money and;
- help the NHS support broader social and economic development.
NHS South West London Integrated Care Board decides how the South West London NHS budget is spent and develops plans to improve people's health, deliver higher quality care, and better value for money.
