Job summary

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

These posts are designed to provide Primary Care cancer specific clinical support and leadership for transformation and improvement of cancer pathways across SEL, supporting the SELCA programme. The post holders will work closely with the wider SELCA team, the Integrated Care Board (ICB), the borough level Cancer CCPLs and wider ICS colleagues as needed.

The postholders will play a key role in driving the cancer transformation agenda and be an advocate for innovation and improvement. Working closely with colleagues across the healthcare system, they will ensure that a Primary Care clinical voice is heard and represented in the development, design and implementation of initiatives and new models of care, with a particular responsibility to ensure that decision-making has strong Primary Care input. They will also emphasise the inclusion of patient perspectives in the development of care models, ensuring that the patient voice is not only heard but is a guiding force in the transformation process. They will have strong interpersonal and relationship management skills, and will be able to work effectively with clinical and managerial colleagues across the system.

Main duties of the job

The postholders will support leadership, development and implementation in areas such as:

o Early Diagnosis - with responsibility to provide clinical leadership at a strategic level on specific programmes of work, particularly primary care pathway priorities as set out in the SELCA delivery plan.

o Personalised cancer care and patient experience priorities within the SELCA delivery plan, including supporting rolling out patient-initiated follow-up, and psychosocial care workstreams.

o Flexible input to other workstreams and projects within the SELCA delivery plan which involve primary care, such as Targeted Lung Health Checks and Faster Diagnosis workstreams.

o Input to the SELCA annual planning process, including identifying priorities and developing workstreams, within the framework of the National Cancer Programme planning guidance.

o Acting as cancer leaders across SEL, working confidently with both Primary and Secondary care to ensure that quality of care and patient experience is prioritised.

o Input into the development of or changes in cancer clinical guidelines for SEL

o Work closely with the borough level CCPLs and Primary Care Cancer Facilitators to drive forward SELCA and ICB Cancer priorities.

  • Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area.
  • Evidence of continuing professional development.

Knowledge

  • Understand the challenges of health inequalities and principles of identifying and addressing these.

Desirable

  • Understand the principles of population health management as applied to addressing health inequalities.
