Job summary

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

The Clinical/Care Professional Lead for Urgent / Unplanned Care is a key post within the Lambeth Together Care Partnership, working across all partners including the NHS (including primary care), Local Authority, and our communities.

The postholder will work collaboratively with a number of existing clinical and care professional leaders including those from; the Acute Provider Collaboratives, Specialist Networks, colleagues and counterparts in neighbouring Southwark Place, SEL ICS colleagues and other stakeholders including region and national in supporting and providing clinical input into realising improvements around urgent / Unplanned Care access and pathways, whilst ensuring the views of colleagues and peers are heard and taken into consideration when delivering improvements and change.

Main duties of the job

The individual will need to work in partnership with other clinical and professional leaders and relevant managers in the Lambeth system and Southwark (as Lambeth leads on UEC for both Lambeth and Southwark), therefore it is key that the Lead works collaboratively with the Southwark CCPL for urgent care and those CCPLs covering primary care. Working with partners in SEL will be part of the remit including engaging with local federations and out of hours providers.o Attend and provide clinical input into the Lambeth and Southwark UEC Delivery Boardo Inform and support local implementation of national requirementso Input into PCN development that focus on meeting urgent and unplanned care needso Actively seek opportunities to improve overall service delivery and performance
