Job summary
Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.
The primary responsibility of the START WELL CCPL To provide strategic clinical leadership to improve the health and well-being of babies, children and young people in Greenwich. The role will include providing expertise and ongoing support to the development and implementation of programmes designed to improve outcomes and reduce inequalities for children and young people.
This will support meaningful integration of primary, community / education and secondary care services, to meet the ever-growing need within our CYP Greenwich population.

Main duties of the job
o Providing pro-active strategic clinical leadership (with key partners and key individuals) for improvements in health and well-being for CYP. This will involve supporting a Population Health Management approach to identifying health and care priorities for children, young people and maternity services, and working with relevant agencies in implementing population health management approaches.o Establishing a strong and broad network of clinical and care professionals working in Greenwich across primary, community / education and secondary care services.o Support for the development, implementation, improvement and evaluation of care pathways for CYPo Improving the health and wellbeing of children and families by reducing inequalities in access and outcomes
