Job summary
Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.
The primary responsibility of the AGE WELL CCPL will be to provide strategic leadership, operational insight and clinical input to our ageing well strategic priority. This will support meaningful integration of primary, community and secondary care services, to meet the ever-growing need within our Greenwich population.
The role also holds responsibility for QUALITY, a critical enabler to our health & wellbeing plan.

Main duties of the job
o Facilitating the development of a clinical vision for ageing well in Greenwich, making the Fuller vision a reality in Greenwich, through better joining up care and improve population health outcomes through to end of life.o Providing clinical leadership to redesign and better integrate how on the day, primary and urgent care demand is best managed across the whole system in Greenwich covering the primary care, urgent care centres, NHS 111 services, out of hours GP visiting services and maximising the potential of enhanced access, Assistive Technology Enabled Care, Pharmacy First.o Providing clinical leadership for the development of our Home First model, which includes frailty, working with partners on joining up the approach for enhanced health in care homes, frailty teams, Joint Emergency Team and Virtual Wards.
