Bexley Clinical and Care Professional Lead for Frailty

Job summary

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

As Frailty Clinical Care Professional Lead, your primary responsibility will be to provide cross-system, strategic and operational leadership and clinical input in delivering the Bexley Wellbeing Partnership 3-year Integrated Plan for Frailty. This is one of our four strategic priorities for the Borough and seeks to improve the health and care for our population as they age so residents can remain well in their homes for as long as possible.

The role will involve providing clinical leadership across our care homes work, dementia care, support for our carers, appropriate use of acute services and ensuring people can die with dignity at the end of their life. You will lead the development of this programme of transformation, ensuring evidence-based practice is adhered to optimising the health and wellbeing for our older population. You will ensure that there are clear lines of communication with all Bexley partner teams to increase knowledge and skills surrounding frailty. You will apply your knowledge and skills from providing hands on care, to inform the redesign, join-up and future commissioning of frailty services.

Main duties of the job

We are therefore seeking a clinical professional who is currently working in a front-line, patient facing role in the Bexley Healthcare System who is dedicated to taking on this leadership role 2 sessions per week on a fixed term contract of employment or secondment. This individual needs to be passionate about improving patient care through working collaboratively with other service provider partners to make the strategic ambitions of more integrated care, an operational reality in Bexley.
