Job summary

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

The Clinical/Care Professional Lead for Primary & Community Care (including Estates Development) is a key post within the Lambeth Together Care Partnership. The postholder will collaborate with the Lambeth Primary & Community Care and Estates teams, and the members/organisations represented by the Lambeth Clinical Cabinet, to provide leadership on a range of associated workstreams.

Main duties of the job

o Acting as a key interface with local and SEL ICB teams, supporting Lambeth to discharge its delegated responsibilities around primary and community care, and estates developments, locally to the Lambeth Together Assurance Group and Lambeth Together Care Partnership Board. This will include playing a lead role at the local Primary Care Forum and Lambeth Estates Forum.o Providing clinical/care professional leadership and guidance on associated key issues and projects locally - e.g. Practice mergers/dispersals, transformation and PCN Development, primary care commissioning, and local out-of-hospital estates strategies and projects.o To form part of the collective decision-making and strategic direction-setting of Primary Care in Lambeth as a full member of the Lambeth Clinical Cabinet, attendee at the Lambeth LMC (Part 2), SEL Primary Care Leadership Group, and part of ongoing engagement with the Lambeth LPC and Community Pharmacy leaders.o To provide a clinical/care professional voice into overall strategic work around out-of-hospital estates development in Lambeth - including helping to deepen relationships with colleagues at the London Borough of Lambeth, GSTT and others.

  • Postgraduate degree or higher professional qualification
  • Evidence of continuing professional development
