Childrens and young peoples Continuing Care,
covering Buckinghamshire, Oxfordshire and Berkshire West (BOB) are looking for
assessors with a range of clinical skills to support assessments of Children
and young people with varying but complex health needs.
We are a team, keen to make a difference with plenty of
experience and knowledge. There will be ample opportunity to develop new skills
working alongside existing learning disability, mental health and physical health case managers leading the
way in the application of Childrens Continuing Care Framework to support
equitable, good quality health care delivery and positive health outcomes.
Main duties of the job
If you have experience in complex physical
health and /or mental health and/or learning disabilities, you may be just the
person to join our thriving team. We are looking for individuals who are motivated
to use their clinical expertise to make a positive difference, delivering
assessments using the National Framework and seeking CYP and family based
solutions often through Personal Health Budgets. As we cover a large
geographical area there will be opportunity to have a primary base in one of
our counties however there will also be an expectation that work is carried out
across BOB in answer to referral receipt and clinical need. Our current
assessors primarily work virtually however there is an expectation that face to
face assessment and visits to home and school are part of each assessment.

and Reviewing individuals within or referred into CYPCC.
The post holder will work with wider teams to support the
delivery of individualised care delivery including Personal Health Budgets
(PHBs).
Working within the remit of the National Framework (NF) for CYPCC,
the post holder will provide a high quality service that supports the delivery
of an effective and competent service that consistently delivers an
individual-focused intervention with patients, their families and
representatives.
Take a lead on ensuring transition
processes are continually developed and improved to achieve seamless CYP key
transition milestones.
The post holder will be required to have a good knowledge of health
and social care practice and be able to demonstrate an understanding of CYPCC
and be prepared to extend their skills, knowledge and competence in this field.
This post will require good communication skills with
individuals and their families or representatives and requires the ability to
work in a multi-disciplinary way with colleagues.
Deliver against objectives, achieving quality outcomes,
prioritising own workload and working to tight deadlines.
The post holder will be responsible for ensuring their
professional registration and mandatory training is up-to-date.
