The All Age Complex Continuing Care Service (AACCC) are looking for a conscientious leader to join our team.

Main duties of the job

The post holder will be working within the remit of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care, and will manage a team of CHC Clinical Practitioners, who undertake a range of tasks within the CHC pathway, including case management, working with Personal Health Budgets, and carrying out assessments and reviews for individuals applying for Continuing Healthcare, Fast Track and Funded Nursing Care payments. The post holder will be required to support the delivery of an effective, competent and person-centred service.

The post holder will support the senior leadership team and their own team to ensure that their workstream of programmes are planned and managed effectively and take the lead in assisting in their successful delivery.

Job responsibilities

The post holder will manage a team of health and care practitioners who undertake a rage of tasks within the CHC pathway including Fast Track and DST Assessments, Case Management, Personal Health Budgets, Eligibility Reviews and Funded Nursing Care Assessments & Reviews.

Working within the remit of the National Framework (NF) for Continuing Healthcare and NHS-funded Nursing Care (CHC) the post holder will manage local team(s) to support the delivery of an effective and competent service that consistently delivers an individual-focused service.

The post holder will be responsible for ensuring the operational teams() practice is compliant with national policy, best practice and the NF for CHC.

The post holder will support the Head of Department and their team to ensure that their workstream of programmes are planned and managed effectively and take the lead in assisting in their successful delivery:

Manage the Teams business support function, including developing and managing the teams business plan, progress and reporting risk and issue management.

Liaise with appropriate functions within the Cluster and the Sector.

Manage all corporate business returns, including planning and workforce returns.

Develop and implement a content management system to ensure information is properly managed and best practice is shared across the team, Directorate and the wider NHS organisation.

Take a lead in the coordination of training & development and recruitment activity across the Team.

Oversight and management of all aspects of the Team budgets.
