Job summary
Are you interested in an exciting
opportunity with the Continuing Healthcare (CHC) Service in Berkshire West area
of Buckinghamshire, Oxfordshire and Berkshire West ICB.
We are looking for enthusiastic
professionals with nursing, social work or allied health professional
qualifications with drive and passion to join our CHC Service and undertake
quality continuing healthcare assessments for people with long term health conditions.
The CHC Service
works core office hours but flexible working is available following discussion
with your line manager. Our office is in Reading, however we are agile working,
with a hybrid mixture of home/office.
Posts are available on a permanent or
secondment basis for a minimum of 12 months. If you are interested in applying

Main duties of the job
This important role involves working
some of the most vulnerable members of society.
The role is central to the delivery of Continuing Healthcare across
Berkshire West through collaborative working across disciplines and professions
with a primary objective of ensuring fair access to CHC with better outcomes
and better experience for our patients.
CHC Services assess and manage a
diverse number of patients with long-term conditions and provides excellent
experience to professionals wishing to extend their knowledge of long-term
conditions, quality assessment and risk assessment processes as well as
determining care packages and ongoing case management requirements.
Working
in a CHC Service equips professionals with knowledge and experience that will
be invaluable to them in their future careers.
This Service is part of the wider BOB ICB where there is currently a
transformationprocess underway to ensure similar working
practices across the ICB so there may be wider opportunities within ICB.

Establish and maintain communication networks to
support care home staff, community staff and hospital multi disciplinary teams
through the process of NHS Continuing Healthcare/funded nursing care
assessment.
To implement systems and processes for NHS Continuing Healthcare funded nursing
care, assessments and reviews across the locality.
To
undertake high quality holistic assessments of patients, clearly identifying
needs using assessment tools as required, as delegated by
Team Leader.
To
work as an integral member of the Continuing Healthcare Team.
To
participate in joint assessments/reviews with Social Services as required.
To
audit screening assessment and decision making tools.
To
make recommendations regarding eligibility to the Continuing Healthcare Team
Leader for discussion at clinical panel meetings.
To
prepare and present these cases to decision making panels.
Review
the suitability of existing placement, care packages and funding and propose change
where necessary.
To
maintain effective communication on issues in complex situations arising from
the application of NHS Continuing Healthcare criteria and the outcomes,
including a range of inter-organisational professionals including GPs and
Health Professionals, patients and families and providers as appropriate.
To
prepare and manage appeal cases for current appeals and restitution cases for
Independent Review panel relating to the application of the NHS Continuing
Healthcare funded nursing care criteria and assessment processes.
To
assist in continual monitoring and analysing information against quality
standards, advising and facilitating agreed proposals for reducing clinical
risk and improving clinical care through the use of clinical systems.
To
maintain data providing locality updates to the manager to support reports for
presentation through the management structure to the ICB.
To
maintain an effective audit trail.

maintain accurate records on agreements.

liaise with Clinical Commissioning, responding to requests for appeals using
