Job summary
An
exciting opportunity has arisen to join a team of pathway and system
integration leaders as part of the ICB in Leeds. The teams purpose is to
create and draw on a matrix of system experts to help integrate services,
pathways, and resources around the needs of populations, improving health
outcomes and reducing duplication.
The
main focus of the role will be people with mental health needs, people with a
learning disability and those with neurodivergence. The role will be part of a wider team which
also covers childrens services. You will collaborate on programmes across a
number of key focus areas both within and outside of this portfolio, as
determined by system needs and prioritisation.
The
post holder will manage a portfolio of services and pathways delegated by the
Programme Directors in line with agreed priorities.
They
will lead and participate in matrix teams across the Leeds Health and Care
Partnership to enable the improvement of services and outcomes for agreed
populations. The post holder will develop and deliver programmes in support
of the Partnership priorities.
The
post holder will work effectively with colleagues across Leeds and West
Yorkshire to ensure developments are well aligned.
The
proposed interview date for this job is 14th Feb, unless a suitable candidate is
identified from the at risk register who has priority due to their
redeployment status.
Main duties of the job
To
support the Partnership in improving the integration of services across
providers and disciplines to improve patient experience
To support the Partnership in
developing a more targeted approach which enables resources to be geared
towards where they will have greatest impact on health outcomes
To support the Partnership in
developing a continuous quality improvement approach
Lead and monitor transformational
projects and programmes and drive transformational change, service
improvement and innovation
Work effectively with clinical
leaders to design and deliver key projects, including developing outline
proposals and full business cases
Provide guidance, oversight and
report on performance for all activities ensuring that targets are
established, achieved and maintained.
With support from colleagues,
analyse a wide range of highly complex data sources, identify gaps in service
provision and develop strategies to tackle unmet need.
Prepare reports and deliver
presentations, to inform and influence service developments and practice
Provide knowledge and leadership in
translating relevant national, regional and local policy
Help enhance working relationships
across partner organisations
Develop and embed an approach of
measurement for improvement for relevant service areas
Working with partners, undertake
best value reviews of services to ensure lean, efficient and cost effective
services.

health problems, and their physical and mental health outcomes. You will also be responsible for overseeing
our work for people with a learning disability, although most of the detailed
management of this programme is carried out jointly with the LA. You will
help drive improvements in our services for people needing diagnosis and
support for Autism/ASD/ADHD and their wider health outcomes.

post holder will provide expertise and leadership to the system both in
regards of specific contracting and service provision in these areas, and in
terms of strategy and direction, advising and reporting to senior leadership
on direction of travel and progress and coordinating efforts to tackle health
inequalities across multiple population segments.
The
post holder will be confident in data analytics and will be capable of
identifying and driving improvement based on both qualitative and
quantitative data.
The
post will be a key liaison point with the WY MHLDA Programme Team, and be
expected to contribute to WY wide work and provide a Leeds voice in this
space. .
The
post holder will have the skills and behaviours to develop strong
relationships across a wide range of partners including the VCSE and Leeds
City Council. They will have the resilience and tenacity to navigate these
relationships in the current financial context, preserving and promoting
partnership working whilst delivering financial sustainability and best
value.
The
