Job responsibilities
The
post holder will assist the ICB in delivering its responsibilities with
regard to the National Framework for Children and Young Peoples Continuing
Care.
To assess Children and Young People
with mental health needs and or learning disability for Continuing Care
working closely with education and social care.
To complete evidence-based
assessments.
To determine eligibility for
Children and Young Peoples Continuing care and carry out reviews of care
packages.
To work with the team to coordinate
personal health budgets and implement the Care pathway for Leeds Children
with complex health needs.
Main
Duties and Responsibilities
The
Post holder will support complex case management and the assessment of
children and young people with emotional and challenging behaviours and/or
learning disability and work in collaboration with other professionals.
To provide high-quality, integrated
assessments for children and young people across Leeds that will determine
eligibility for Childrens Continuing Care.
To use clinical expertise to
analyse and challenge health information and assessments to ensure that an
accurate assessment of childrens needs are documented.
To support implementation of the
personal health budget process.
To communicate effectively with
colleagues, partner organisations, service users and their representatives.
To maintain a rapid and effective
response to requests for advice and assessments.
To work within the timescales
outlined in the Childrens Continuing Care Policy.
To work with colleagues to
implement the care pathway for discharge of Leeds children with complex
needs.
To meet statutory duties under the
SEND Code of Practice, engaging in the Education, Health, and Care planning
process to ensure alignment of EHCP and Continuing Care.
You will be expected to demonstrate
knowledge and understanding of all aspects of safeguarding children and work
within organisational policies.
All staff are expected to work
flexibly across all functions according to the skills and requirements for
individual roles.
Support development of policies and
protocols as appropriate.
Lead on a specific project or area
of work as directed by the Manager, deliver outcomes with a managed approach,
providing timely updates as required.
Lead and co-ordinate delegated
projects to deliver agreed outcomes and report progress as necessary.
Day to day leadership and line
management, support setting direction of the team ensuring all team members
have clarity of purpose.
Produce timely function specific
advanced reports, papers and analysis using prolonged concentration where
required.

Provide timely advice, information
or support to relevant personnel internal and external around childrens
continuing care and personal health budgets, adhering to policies,

of negotiation, persuasion or motivation skills where appropriate as part of
the role.
Handle information, which may be
sensitive, complex or confidential and appropriately recording, transferring
and/or co-ordinating such information in accordance with the Data Protection
Act.
Make judgements on a range of
issues relating to childrens continuing care and personal health budgets,
using own expertise and within responsibility level.
Plan and manage own workload to
weekly, monthly and annual timescales to ensure continuity of service.
Plan and co-ordinate meetings as
appropriate.
Provide day to day supervision of
support staff ensuring all policies are adhered to and agreed objectives are
monitored. Attend regular supervision and demonstrate development through the
supervision process.
Communication

The
post holder will be expected to communicate with a wide range of people, at
all levels from different organisations, as well as families and carers and
those who have an interest in Children's and Young Peoples continuing care.
Communication
will include verbal and written, as well as presentations, education, and
training.
