Job summary

LeDeR is a service improvement programme which aims to improve care, reduce health inequalities, and prevent premature mortality by understanding the health and social care support people received and applying a learning approach. It is one of the key programmes overseen by the ICB and funded by NHS England and NHS Improvement. The programme was developed in 2017 and is guided by a national policy and training. West Yorkshire ICB has made good progress in the implementation of the LeDeR Programme policy and has established strong partnership working with partners and we are now at a stage where we are starting to influence change to enable better health outcomes for people with a learning disability and autism.

Main duties of the job

To complete high quality reviews efficiently and within national timescale requirements and where necessary co-ordinate multi-agency reviews which may be of a complex nature

Develop resources to support Reviewers to conduct quality reviews and enable Reviewers to adequately support bereaved carers and families with whom they come into contact through the course of undertaking a review

Ensure that Safeguarding principles are considered and applied within all elements of the LeDeR programme and establish links with key local Safeguarding contacts and ensure that any concerns are reported as per Safeguarding Children and Adults policies.

Identify the most appropriate evidence available including the conversation with the family or loved one, conversations with the GP or GP records and other health and care data sources and using high level professional judgement and experience to determine and agree with senior reviewer, whether a review proceeds to a focused review.

Explain the parameters of the LeDeR review, signpost to other processes such as complaints processes, and discuss with the family their expected outcome of any LeDeR review setting realistic expectations at the start.

Present review findings at governance panels and actively participate and support the panel to define SMART actions offering professional challenge and proposing solutions to address recommendations identified in completed LeDeR reviews.

Job responsibilities

The purpose of LeDeR is to identify any potential learning from the care of the person who has died, identify any avoidable factors that may have contributed to a persons death and ways of improving health and social care services for people with Learning Disability in the future, in order to reduce premature deaths.

Supported by the Senior Reviewer , the post holder will be responsible for working collaboratively with our provider colleagues (across the health and social care sector) to embed the LeDeR programme into everyday practice.

In addition to the LeDeR programme, the post holder will be required at times to support the Quality Team in the achievement of their goals to secure improvements in health outcomes, reduce health inequalities, and improve patient experience.

Main Job Responsibilities

Function Specific Responsibilities

Support the embedding and monitoring of a locally agreed LeDeR process, working collaboratively with colleagues across NHS Trusts and Local Authority to deliver the LeDeR programme.

Build and maintain relationships across the health and social care sector.

Identify when cases have involvement with other investigators (eg: HM Coroner, Police, Safeguarding, NHS Serious Incident process) and discuss appropriateness of LeDeR review.

When applicable support the facilitation of multi-agency reviews of deaths

Provide a range of robust LeDeR monitoring reports highlighting areas of good practice and areas of concern. Present reports to the WY ICB Quality Committee when required.

Collate findings of reviews and lessons learnt.

Play a key role in focussed West Yorkshire ICB led initiatives i.e. Think LD approach

To create a culture which values openness, honestly, candour and challenge.

To act as champion for people with learning disabilities, ensuring that people with a learning disability and their families and carers are a key part of the LeDeR process.

Committed to improving service provision for people with learning disabilities and their families.

To uphold organisation policies on the promotion and principles of equality.

Work on own initiative, taking advice from manager as required; recognises their work is assessed regularly and embraces the positive aspects of this.
