Job summary
The
All Age Continuing Care Service in Leeds seeks a full-time Senior Clinical
Operational Manager to lead the Adult Clinical Team.
This
exciting and challenging role will provide clinical expertise to the
Continuing Care Service, including clinical leadership, day-to-day
operational management, and performance management of the clinical team. They
will also be the expert clinical user for the digital system used across the
service, embedding the system within clinical practice.
We
seek a dynamic individual with excellent planning and communication skills
who can work autonomously, prioritise effectively, and deliver targets within
a set deadline.
They
will work with the Senior Operational Lead to ensure the data available
across the service is of high quality, respond to Freedom of Information
requests and answer complaints.
The
ideal candidate will have a strong clinical background, in-depth knowledge
and practical experience in delivering Continuing Care and Continuing
Healthcare. They must be able to assess and plan to mitigate risk, plan care
delivery and support, and will be required to negotiate with care providers,
clients, and their representatives professionally.
The
post holder will be on a rota to provide on-call cover for service queries
(currently 7 days 1/4 weeks).
Main duties of the job
The
post holder will be responsible for the line management and senior leadership
of the clinical team. They will ensure the delivery of NHS Continuing
Healthcare and Continuing Care for Children and Adults and provide support
across all areas, including physical disabilities, mental health, older
people, learning disability and autism.
They
will ensure that the process for assessments, reviews and appeals are carried
out in accordance with the National Framework for NHS Continuing Healthcare
and Funded Nursing Care, and performance of staff within the team is well
managed.
The
post holder will retain a key senior clinical presence and function in the
team, ensuring that systems and processes are in place to maintain a high
level of clinical skill and competence to support effective evidence-based
assessment and decision-making and processes to ensure the equitable
allocation of resources.
They
will build and maintain close working relationships with the acute hospitals,
developing and maintaining discharge to assess models of care, and safe and
effective discharge pathways for those at end of life and eligible for Fast
Track CHC support, as well as strong links with other NHS and independent
providers across the area.

Service, resolving challenging operational issues whilst maintaining the
service projections.
