Job summary
We
are looking for a proactive and highly motivated Advanced Pharmacist to join
the West Yorkshire Integrated Care Board (Leeds) Medicines Optimisation Team.
We
welcome full and part time applicants on a permanent basis.

Main duties of the job
To
work as part of a multi-disciplinary team within the Leeds Health and Care
system in leading, developing promoting and implementing the high quality,
evidence based and cost-effective use of medicines in a way that maximises
the benefits of medicines to patients and service users and which minimises
risks (clinical, legal and financial).
To
work with other Health and Social Care agencies across Leeds, to implement a
co-ordinated medicines optimisation approach across the wider health
economy.
To
develop, support and implement agreed prescribing and pharmaceutical
activities or programmes at Leeds, PCN level or where necessary at individual
GP practice level.
Actively
works towards developing and maintaining effective working relationships both
within and out with the ICB, Leeds GP Confederation and West Yorkshire
Integrated Care System.
Fosters
and maintains strong links with all services across the city.
Explores
the potential for collaborative working and takes opportunities to initiate
and sustain such relationships.
Uses
negotiation and influencing skills to collaborate with a wide range of
stakeholders.

Job responsibilities
To
lead and coordinate medicines optimisation services and clinical pharmacy
development according to local, regional and national strategy to improve
quality of care, personalise care and patient experience.
To
provide clinical leadership to ensure high quality and cost-effective
prescribing.
To
identify, plan, organise, and co-ordinate the implementation of medicines
optimisation initiatives, objectives and complex changes to bring about quality
improvements in prescribing behaviour and better outcomes for patients
including reducing patient harm & improving medication safety.
Lead
the development & implementation of new medicines optimisation
initiatives and services within GP practices, community pharmacies and
community providers. This includes the development of prescribing / medicines
optimisation guidelines and protocols.
Work
autonomously with significant discretion and work collaboratively with a wide
range of internal and external stakeholders.
To
analyse and interpret complex information and communicate this information
using a variety of methods to patients, carers, and other health
professionals to promote the safe, high quality & cost-effective use of
medicines in all prescribing decisions. This information includes clinical
information regarding patient care, technical therapeutics information and
prescribing activity data derived from clinical audit data, ePACT2 and
similar sources.
To
provide specialised advice and support in risk management, clinical
governance issues and ensure compliance with medicines legislation, patient
safety advisory notices and other established good practices to manage and
minimise risk.
To
maintain and develop professional competence and expertise, keep up to date
with medical/therapeutic evidence and opinion, and local and national
service, legislation, and policy developments, agree objectives and a
personal development plan and participate in the appraisal process.

- Professional knowledge acquired through vocational masters degree in pharmacy (4 years) or equivalent + 1 year pre-registration training
- Registrant of the General Pharmaceutical Council.

- Extensive knowledge of medicines optimisation, acquired through post graduate clinical pharmacy diploma or equivalent experience or training.
- Understanding and knowledge of the workings of the NHS, the challenges it faces and of the reform agenda.
- Evidence of significant personal development, professionally and managerially
- Experience of working as part of a team and independently.
- Experience of communicating and engaging effectively with internal and external agencies/stakeholders.
- Experience of planning complex medicines management projects and programmes
