Calderdale

Reporting
to the Deputy Director (All Age Acute and Community) in the WY ICB
(Calderdale place) you will work in a supportive, dynamic team providing
expertise and taking innovative approaches to transform childrens physical
and mental health and care. You will work alongside the Designated Medical
Officer (DMO)
Main duties of the job
The
successful post holder will:-
have subject matter expertise and
programme management skills to lead system change across childrens health
and wellbeing in Calderdale & West Yorkshire;
be a decisive and action orientated
problem solver;
be resilient, flexible and
adaptable, working in a multi-faceted programme as part of local, regional
and national systems;
lead a range of projects within a
matrix environment;
build & maintain strong
relationships, working collaboratively with parent carers, children, young
people & system partners in Calderdale and West Yorkshire;
ensure the Healthy Children and
Young Peoples Priority Programme and associated priorities are delivered for
children & young people aged 0-25 years who have Complex Needs;
align plans for children and young
people across Calderdale to ensure those with complex needs have a better
start, achieve better outcomes and have an improved quality of life;
act as strategic & operational
health lead for the Education Health and Care Plan (EHCP) planning process
and pathway;
lead the design & delivery of
quality improvement in SEND, mental health, social emotional mental health,
learning disability, neurodiversity, and physical health and disabilities;
facilitate the delivery of
measurable, sustainable, value-for-money improvements that improve children
and young peoples outcomes, impact and experiences;
work flexibly to achieve the best
balance between home and work life.

Working

SEND will provide the strategic leadership necessary to ensure that WY ICB
(Calderdale Place) and the health organisations it commissions services from
meet the statutory duties of the Children and Families Act (2014) and the
SEND Code of Practice (2014) for children and young people with special
educational needs and disability, aged 0 25 years.
In
addition, the post holder will work closely with children, young people,
parents, families and carers and strategic Partners across health, education,
social care and the voluntary sector (the integrated care system) to ensure
that the Healthy Children and Young Peoples Priority Programme and
associated priorities are delivered for children & young people aged 0-25
years who have Complex Needs. This includes ensuring the alignment of plans
for children and young people across the Calderdale Cares Partnership for
special educational needs and disability, mental health, social emotional
mental health, learning disability, autism and physical health and disabilities
to ensure that children and young people with complex needs have a better
start, achieve better outcomes and have an improved quality of life.
The

individual, organisational and strategic outcomes for children and young
people through a personalised, child centred approach.
Be educated to masters level or
equivalent level of clinical experience, working at a senior level which
includes SEND/ Complex Needs as a specialist area (or equivalent years
experience),
Have a professional qualification
as a health professional (occupational therapist, speech & language
therapist, physiotherapist or registered nurse with experience of working
with children and young people/ SEND/Learning Disabilities, Neuro-Diversity,
Complex Needs).
And have significant clinical
experience as professional in the field of SEND and/or Complex Needs, with an
in-depth knowledge and understanding of the legal framework, policies and
practice related to the Children and Families Act 2014, Care Act 2014,
continuing care frameworks, and SEND Code of Practice 0-25 (2015).
The
candidate will be a strategic thinker, pragmatic and able to work on their
own initiative. They will be confident and experienced in delivering system
change through involvement of people within their team, ICB, multi agency
partners and those with lived experience.
