The
NHS West Yorkshire ICB, Bradford District and Craven locality is seeking to
recruit a highly motivated, enthusiastic and forward thinking clinical
assessor to work as part of our Continuing Care team. The successful
candidate will be working across our varied and diverse population. You need
to be able to work autonomously and collaboratively within a multi-agency
environment and dynamic skill-mixed team.

Main duties of the job
As
a Continuing Care Assessor your role will include assessing individuals
against the National Framework for NHS Continuing Healthcare and Funded
Nursing Care. The role has various aspects including triaging new referrals,
travelling to see patients to complete assessments and reviews, and liaising
with families and other stakeholders.
As
part of the review process, you will review the eligibility for the NHS CHC,
as well as the care planning, to ensure that all care needs are being met
safely and effectively. You will monitor and influence the quality of care
that is provided to patients; drawing on previous skills and knowledge to
improve the patient experience of care. You will provide professional advice
to patients, carers and multi-agency colleagues, including signposting to
appropriate services and resources.

The post holder will use
experiential knowledge and clinical skills to ensure effective application
and interpretation of the National Framework for the NHS Continuing Health
Care and NHS Funded Nursing Care.
The post holder will work closely with
hospitals, community services and care providers to develop systems and
processes to facilitate a smooth assessment and referral process under the
supervision of the Senior Nurses.
The post holder will complete
non-complex decision support tool meetings and formulate recommendations for
panel consideration.
The post holder will regularly
review, under the supervision of the Senior Nurses, the clinical needs of the
patient with regard to appropriateness of the placement, level of care being
provided and entitlement to NHS Funded care or Continuing Healthcare.
The post holder will require good
verbal and written communication skills to communicate with patients, their
families and other professionals. The production of accurate, detailed and
comprehensive reports is essential.
Complete CHC and FNC Nursing Needs
assessments, reviews and checklists for patients in accordance with the
National Framework for NHS Continuing Health Care and Funded Nursing
Care.
Complete assessments as required
for appeals or retrospective work as applicable.
To support the duty nurse function
as required and undertake other duties at the request of the line
manager/supervisor, which are commensurate with the role.
Communicate and assist colleagues
and stakeholders around issues within own area of work.
Communicate and co-operate with
internal and external staff, patients, carers, and families.
