Job summary
An exciting opportunity has arisen for an appropriately skilled and experienced registered nurse to join Kirklees Health and Care Partership's Continuing Healthcare (CHC Team) as a lead nurse for the learning disability and mental health CHC Team. Although direct experience of working in a CHC team is not essential, candidates must be able to demonstrate knowledge and experience of the CHC legislation and process and have experience of working within either the MH or LD field of practice.
Assessment, including risk assessment, and case management skills are essential.
You should have excellent communication and interpersonal skills with the ability to produce clear and concise written reports.
Full-time and part-time hours considered.

Main duties of the job
The post holder will be responsible for undertaking NHS Continuing Healthcare/ Funded Nursing Care assessments, care planning and the evaluation of packages for patients eligible for NHS Continuing Healthcare. They will support complex case management and work in collaboration with other health professionals and social care staff. The post holder will deputise for the band 7 nurses. The post holder will have responsibility for agreeing eligibility and will make financial decisions within the NHS Kirklees Health and Care Partnership standing financial instructions.
