Job summary

An exciting opportunity has arisen for an appropriately skilled and experienced registered nurse to join Kirklees Health and Care Partership's Continuing Healthcare (CHC Team) as a lead nurse for the learning disability and mental health CHC Team. Although direct experience of working in a CHC team is not essential, candidates must be able to demonstrate knowledge and experience of the CHC legislation and process and have experience of working within either the MH or LD field of practice.

Assessment, including risk assessment, and case management skills are essential.

You should have excellent communication and interpersonal skills with the ability to produce clear and concise written reports.

Full-time and part-time hours considered.

Communications & Relationship Skills

Undertaking Continuing Healthcare (CHC)/Funded Nursing Care (FNC) assessments and reviews for patients across all aspects of care. This necessitates a high level of assessment skills requiring tact, diplomacy and the ability to liaise effectively with colleagues, health care professionals, service users' carers and family members.

Offering continuing support and advice to family and carers in complex and sensitive situations, necessitating a high level of interpersonal, and reassurance skills; supporting the local resolution process in the case of complaints/appeals/retrospective cases and providing information on a timely basis.

Developing and maintaining effective and appropriate communication networks with health and social care professionals and act as a resource to them.

Working with relevant health professionals, CHC colleagues, Business Support Staff, Senior Leads and other Support Staff within the CHC team to ensure a seamless and efficient service is delivered.

Analytical & Judgmental Skills

Exercising professional judgement as necessary when analysing and communicating complex and sensitive information on behalf of the CHC service and application of the associated criteria.

Interpreting national and regional guidance relating to the implementation of the CHC National Framework and subsequent developments within the CHC service.

Decision making with regards to multi-disciplinary team CHC assessments to determine eligibility.

Supporting Community Matrons/District Nurses in their case management roles for complex cases that are eligible for continuing healthcare.

Planning & Organisational Skills

Deputise for the CHC Team Leader when required.

Deliver against organisational objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines.

Plan, organise and implement reviews and assessments, to co-ordinate with Local Authority, family and providers

Patient/Client Care

Ensuring there is a holistic care plan in place, including the production and co production when required with Social Services.

Ensure personal health budgets are actively promoted with service users and their representatives and work closely with the CSU to encourage a seamless delivery

Identify risk of abuse and need for protection from harm to self or others and report in line with local safeguarding routes and governance arrangements.

Ensuring timely "End of life" provision and reviews, following the national guidance for clients Fast Tracked through the CHC system.

Providing specialist nursing advice to secondary care, primary care, nursing homes etc to facilitate the continuing healthcare process.

Policy and Service Development

Ensure policies are implemented and adhered to whilst identifying and proposing changes for improvement to practices and procedures in own area. Required to implement changes to policies and procedures.

Finance and Physical Resources

Approving a package of care in line with the NHS Kirklees Health and Care Partnership standing financial instructions and agreed thresholds.
