Job responsibilities
All senior post holder within Bradford district and Craven HCP (Healthcare Partnership) are expected to be system leaders who manage complex relationships, innovate, work beyond boundaries and serve the people of Bradford district & Craven from pre-conception to their death.
In undertaking your role in the HCP you will:
Work in line with the health and wellbeing strategies for Bradford & North Yorkshire to deliver the Bradford district and Craven vision of happy, healthy at home.
Ensure the HCP works as a key strategic partner in our local system and in the West Yorkshire and Harrogate ICS.
Ensure the HCP delivers its core purpose and functions many of which are set out in law, including but not exclusively:
Meet health needs within the financial allocations
Work with the councils to ensure our population is healthy
Work with strategic partners to design and deliver health, care and support services
Our specific responsibilities for continuing health care, safeguarding and emergency preparedness
Take delegated primary medical care commissioning responsibility
Role Purpose:
The role supports the business in driving transformation as well as value for money in planning, commissioning and services within Personalised Commissioning processes.
The post holder must operate effectively in a flexible and demanding environment and proactively engage with the Independent Sector, NHS and Local Authorities to deliver personalised commissioning services locally and across the country and work effectively with a variety of external consultancies working in partnership on projects and programmes within the NHS social care organisations and independent care sectors as required.
The post holder will focus their efforts on 4 key drivers:

Main duties of the post holder:
Policy and Service Development responsibilities for development and implementation of policy and/or services.
Provide and receive complex, sensitive and contentious information. Requirement to present information to a wide range of stakeholders in a formal setting.
Utilise skills and ability to deal with potentially aggressive / antagonistic situations.
Committed to working and engaging constructively with internal and external stakeholders on a range of business sensitive issues.
Nurtures key relationships and maintains networks internally and externally, including regional and national networks as appropriate.
Link with managers and members of other functions, to address inter-dependencies and ensure alignment.
Apply a structured change management approach and methodology for the impact of any change.
Supports the delivery of day to day activities and projects
Manages the team and drives delivery of a range of business initiatives and projects
Supports the identification and sharing of best practice in employee engagement
Operates in a highly political and sensitive environment.
Finance and Physical Resources Responsibilities of the job for financial resources, e.g. invoice payments, budgets, and physical assets, e.g. premises, patient possessions, goods and supplies.
Have delegated responsibility for specific team budgets and awareness of budget spends within personalised commissioning by the HCP and partners.
Is responsible for the cost effectiveness of services and, working in collaboration, negotiates appropriate costs with providers.
Supports the portfolio of initiatives in demonstrating value for money for the current spend through tracking, managing and delivering agreed benefits
Negotiates with Local Authorities regarding responsible commissioner and percentage splits of care costs, attending funding panels as required and/or supporting the Case manager to do so.
Human Resources responsibilities for management, supervision, training, development of employees and others.
Provide line management to Clinical Leads
Deputise for Head of Personalised Commissioning as required, expanding on knowledge, skills and experience within personal professional development.
Supports the wider team in the delivery of service plans, allocating tasks as appropriate, identifying risks, issues and dependencies, considering best practice and current options and ultimately making decisions in the best interest of the service.
Assist in the development of a comprehensive and cohesive plan for the transition period which is consistent with the overall project timetables, meets the strategic direction of the team and minimises unnecessary disruption to stakeholders involved in the process and is operationally sound.
Pro-actively manages stakeholders, respond to and resolve conflict between different stakeholders when this arises through facilitation or other appropriate mechanisms.
Is responsible for a high standard of work supporting the delivery of the service to any applicable quality standards and in a cost effective manner.
Ensures the flexibility of the service to meet conflicting / changing requirements.
Uses appropriate communication services and communication tools as required.
Demonstrate effective stakeholder management across different departments and at all levels.
Take into account the impact of any changes within the service will have on the local NHS or its partners and work to ensure sure full ownership and buy-in within the team. Advocate on behalf of the service at senior and executive levels and ensure active engagement and sponsorship within NHS as a whole.
Notify the Head of Personalised Commissioning of service risks which subsequently need to be clearly documented in the appropriate format.
Act in a way that is compliant with Standing Orders and Standing Financial Instructions in the discharge of budget management responsibilities.
Oversee and manage the delegated budget for assigned function/team. Responsible for ensuring adherence to the budget, ongoing monitoring of expenditure against budget and ensuring the appropriate documentation is available for scrutiny.
Identify products, equipment, services and facilities for assigned activities, achieving stakeholder buy-in as required. Placing orders and authorising invoices, being mindful of cost.
Is responsible for making recommendations, providing advice and able to provide strategic reports/briefings for Commissioners, Head of, Deputy Director, Steering/Reference Groups and others as required.
Evaluate demands for services and make recommendations to the Commissioners
In the absence of the Strategic Lead/Head of Service, Operational Manager may be required to manage the CHC Clinical Leads and CHC Business Manager who are responsible for day to day work assigned to team.
Responsible for the recruitment of staff, chair of recruitment panels and acting as the recruiting officer.
Responsible for undertaking appraisal and personal development including progressing any disciplinary or capability issues.
Information Resources Responsibilities for information resources, e.g. either computerised or paper based records, or information systems, e.g. medical or employee records.
Forge close positive working relationships, in order to support an effective matrix approach to achieve NHS objectives.
To support, motivate and develop all staff to ensure that they are able to deliver the new responsibilities of the NHS strategy.
Manages third parties to ensure deliverables are met in a timely manner and within budget.
Provide reports summarising status on issues, appraising outcomes, and providing progress reports as requested.
Collate as required, qualitative and quantitative information and lead appropriate analysis.
Collect, Analyse, interpret and present data to highlight issues, risks and support decision making.
The post holder will be responsible for the development of the HCP overarching strategy for commissioning for health, care and support that achieves the system vision of happy, healthy at home. Taking account of:

Core responsibilities:
It is the responsibility of each member of staff to maintain confidentiality at all times.

Safeguarding is everyone's responsibility. All employees have a responsibility to safeguard and promote the welfare of children and adults and to consider how their role in the organisation can contribute to the HCP safeguarding work. The post holder will be responsible for ensuring that they undertake the appropriate level of safeguarding training and that they know how to recognise, seek advice and respond to any concerns of abuse, according to the HCPs policies. This may include concerns related to patients, colleagues or members of the public.
