Job summary

The post holder will be committed to improving service provision for people with learning disabilities and their families through the undertaking of Learning Disability Mortality Reviews (LeDeR) as part of the LeDeR programme. The reviews will be from West Yorkshire and the post is hosted by Bradford & Craven HCP on behalf of the ICBs which also include the areas of Calderdale, Leeds, Kirklees and Wakefield.

Main duties of the job

The main part of the role will be to mentor and guide LeDeR reviewers as well as develop networks across the patch in order to facilitate the LeDeR Process. This also includes involvement and facilitation of the learning that requires embedding as part of the LeDeR process of services to improve the lives of patients with LD and/or Autism and their families.

The Senior Reviewer will work closely with the Regional LAC (Local Area Contact) in this regard. A large part of the role is providing clinical and line management supervision to the Clinical Reviewers, so they continually supported and also ensuring oversight over the focused review panels. This is also done in conjunction with the LAC by proving support to this function of the programme, as well as diligent and regular reporting with the expected governance arrangements on this and liaising with NHS England as appropriate for this statutory function.,

- Reviewing care records for the person

- Identify, share and agree learning across the system

Build and maintain relationships across the health and social care sector

To use the online LeDeR web based platform and comply with the reporting requirements of LeDeR and LeDeR methodology and manage the LeDeR notification process on behalf of the Local Area Contact (LAC)

Assist reviewers in identifying when cases have had involvement with other investigators (e.g. HM Coroner, Police, Safeguarding, NHS Serious Incident process) and agree approach with Local Area Contact in line with LeDeR guidance

Provide a range of robust LeDeR monitoring reports highlighting areas of good practice and areas of concern. Collating findings of reviews and lessons learnt

Provide professional support and mentoring generally to local reviewers across West Yorkshire

Qualifications

Essential

  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
  • Registered health or social care professional e.g. RGN, RNMH, RNLD, Social Worker or Allied Health Professional

Desirable

  • Project management qualification

Essential

  • Awareness of statutory clinical review functions such as SJR (structure judgement review process, inquests, Safeguarding Adult reviews and forthcoming PSIRF/arrangements)
  • Experience of patient/public involvement in services
  • Experience in the of Investigation of incidents/Human Factors
  • Root Cause Analysis trained/Human Factors
  • Experience of wider mortality reviews
  • Working to and achieving tight deadlines
  • Working with or supporting people with a learning disability
  • Working with others using a project management approach
  • Communicating and engaging effectively with external agencies/stakeholders
  • Working closely with multi agencies
