Job summary
An
exciting opportunity has arisen within the Kirklees all age continuing care
team for a Children's and young person assessor and case manager. The role
involves working within a team who specialise in assessing and case managing
Children and young people with identified health care needs.
This
role invites opportunity to work in partnership with the Local Authority and
wider stake holders forming a multi-disciplinary team to ensure robust
assessment and quality care provision for this client demographic within the
Kirklees area.
Main duties of the job
The
post holder will facilitate, manage, and complete children's continuing care
assessments in line with The National Framework for Children and Young People
(2016), working together and in conjunction with the wider members of the MDT
to ensure accurate and robust information gathering in order to inform a
recommendation regarding eligibility for children's continuing care.
The
post holder will be responsible for the identification of an appropriate care
plan outlining person centred care delivery individual to those eligible for
children's continuing care. This will include establishing a robust care
package to meet the identified needs of the patient inclusive of risk management
and mitigation to allow for the best outcomes for an individual and their
family. This will include support with hospital discharge.
The
post holder will be responsible for an allocated case load of children and
young people, being the point of contact for enquiry, providing specialist
advice being proactive and at times reactive to concerns raised to prevent
health deterioration and care package breakdown.
The
post holder will support with smooth transition of a child/young person into
adult services being part of a coordinated team and contributing to the
assessment process.
The
post holder will be required to visit Children, young people and families as
part of the assessment and review process, environment can vary from a
patients own home to an educational facility or hospital.
