Job summary

Every day in Leeds, thousands of people receive great care and support from dedicated health and care staff, volunteers, and from carers. But sometimes people fall through the cracks, and the system can let people down. Too many people spend more time in hospital than they need to, our community provision is fragmented with variable outcomes, and many people could remain independent for longer with different support.

To help tackle these issues, the Leeds Health and Care Partnership (LHCP) have come together to redesign and transform intermediate care across the city. The Intermediate Care Programme aims to develop and implement a new model of intermediate care services to achieve more independent and safe outcomes and help more people stay at home, whilst improving the experience for people, carers, and staff. The LHCP now want to recruit to a Programme Director to jointly lead a dynamic programme team to design, test out, and deliver a new model of care for Leeds.

Main duties of the job

You will work alongside another Programme Director leading a team of project managers, clinical leads, and operational experts across a range of workstreams. The role is hosted by the ICB in Leeds, this is a LHCP partnership role and as Programme Director you will be expected to work closely with all health and care partners across the city to ensure successful delivery of the Programme. You will also work closely along side the external delivery partner for the programme ensuring this resource is maximised for the LHCP.

This role will be offered on a secondment basis and applicants should have agreement from their current line manager before applying. The role will be for a minimum of 12 months and up to 18 months.

To develop, communicate and support a common understanding of the strategic aims of intermediate care within the context of the changing health and social care environment both nationally and within Leeds.

To work partners and stakeholders to deliver and implement the longer term vision for intermediate care across leads in order to meet the changing needs of service users and their carers

To maximise the opportunities for partnership where this provides the long term vision of better outcomes for service users and their carers; avoids duplication and makes the best use of limited resources and improves peoples overall experience of health and social care.

To engage in market analysis to ensure that opportunities to innovate and develop intermediate are maximised.

Communications & Relationship Skills

Uses complex influencing strategies, tailored to individual situations, to build on and develop effective partnerships with key stakeholders. Understands and addresses underlying problems, opportunities or political forces affecting integrated health and social care in relation to the external operating environment, in order to achieve the best possible outcomes for service users.

Strive to create the conditions for successful partnership working, across the system developing an effective interface with a wide range of partner organisations, ensuring relationships are developed and maintained in order to enhance the patient experience, optimise the use of public sector resources and ensure ownership of the change

To develop a strong clinical and professional leadership cadre, working closely with clinical and professional leads across the system

To work closely with Local Authority colleagues on managing the political and operational issues and ensure concensus

To develop and promote a culture where service users, carers and the public are involved and feel engaged in the dialogue and plans for integration of health and social care

To develop clear mechanisms for working in partnership with staff so they feel fully engaged and able to influence the provision and development integrated care

Create a team to oversee delivery drawn from all stakeholder organisations

Planning & Organisational Skills:

Ensuring the appropriate governance arrangements, a robust communications plan and a benefits realisation plan are in place including ensuring the appropriate governance arrangements exist within the Transformation Board to oversee decision making and delivery

Manage the specific groups of projects ensuring capability is delivered, risks and issues are registered and logged and escalated where necessary and that the defined outcomes are achieved.

Ensure lessons are learned throughout the programme and that project evaluation reviews take place in order to ensure benefits are measured and realised

Finance and Physical Resources:

Oversight and management of the programme budget

Ensures compliance with Statutory Financial Instructions of the ICB and wider LHCP partners ; ensuring probity and good financial governance.

Human Resources and Leadership

Provides leadership on the intermediate care programme, working across agencies in order to deliver service results, inspiring others to be positive in their support of successful integration. Challenges traditional organisational boundaries and ways of working, emphasising integration and partnership.

Ensure a leadership style which is in line with agreed values, empowering staff to lead from every seat in the organisation.

Effectively builds and maintains relationships with staff and staff side organisations across organisational boundaries.

Manages a team of staff to drive delivery
