Job summary

WY HCP is going through an extraordinary period of change, working in partnership with our 5 local places and across several programmes to support significant transformation. Due to changing roles and evolving portfolios our team is seeking new, dynamic leadership to support the evolution of our team with new thinking and ideas. The area of responsibility is initially Personalisation and Unpaid Carers, but there is opportunity to work right across the Long Term Conditions and Personalisation programmes as we continue to connect pathways and systems.

Main duties of the job

Contributing to the strategic planning of the function, identifying interdependencies across component programmes/projects, potential impacts on wider organisation, resource requirements and building in contingency and adjustments, as necessary

Contributing to the financial management of the function and its constituent programmes ensuring value for money

Ensuring timely and accurate information analysis and reporting to the Associate Director

Providing operational and strategic leadership to support transformation, sharing of best practice and achieving the key performance indicators aligned to all programmes within the function

Working closely with the Accountable Officers, Chief Executives, Clinical Leads, Clinical networks and NHS England providing high quality specialist advice, expertise and leadership on communications and engagement to the function, its programmes and the Partnership

Ensuring any risks are understood and where appropriate mitigated through appropriate actions and strategies where necessary supported by the ICS.

Promoting a culture of effective joint partnership working ensuring that there is clear reporting and escalation of issues when issues are raised providing clear support to places acting as a key link between the Programme Manager and place based delivery leads

Ensuring the principles of openness, transparency and candour are observed and upheld in all working practices

The post holder will apply programme management expertise and best practice in managing a number of programmes and projects in a disciplined manner.The Senior Programme Manager will directly report and support the role of the Programme Director and ensure that programmes of work are well established, coordinated and delivered on time. This is a strategic leadership role which will lead and influence system change working with a range of stakeholders of varying seniority and experience from across the WY & H Partnership including engagement with voluntary sector, local authority, clinical leads, carers and service users, often having to translate complex information to audiences of differing backgrounds. He/she will use their experience to ensure a structured programme management process to their approach, methodologies and principles to deliver positive outcomes and above all, measurable results. There is a need to work collaboratively with other ICS programmes of work to support effective delivery of transformation and change initiatives including establishing roles and responsibilities at the outset and reviewing progress regularly.

The Programme Manager will ensure a robust framework by which to manage, monitor and report on the Unpaid Carers and Personalised Care programmes across West Yorkshire & Harrogate Health and Care Partnership.

The post-holder will be responsible for continuing to strategical lead and support transformation and spread in respect of sharing an best practice and achieving the key performance indicator for both programme across the partnership. The post holder will lead on the on-going development and implementation of agreed monitoring systems in place.

The Programme Manager will work with place leads across WY&H to provide capacity and resource to deliver on the strategy and priorities of the personalised care and unpaid carers programmes

The post holder will work closely with the Accountable Officers, Chief, Executives, Clinical Leads and NHS England providing high quality specialist advice, expertise and leadership on communications and engagement to the Programme and the Partnership.

The post holder will work with key stakeholders in developing effective partnerships to support making demonstrable improvements to patient care on behalf of all organisations within the Partnership.

To ensure an appropriate degree of rigour and discipline in relation to the approach, structure, progress reporting and management of risk within the programme that is proportionate to the devolved (to place) approach to delivery.

To work in partnership with CCGs, NHS Providers and Local Authorities and the wider partnership to ensure that the approach to delivering the Personalised Care programme is clearly articulated and that each place is supported to enable each to ensure the delivery of the key objectives within both programmes and that any risks are understood and where appropriate mitigated through appropriate actions and strategies where necessary supported by the ICS.

Through strategic leadership, promote a culture of effective joint partnership working ensuring that there is clear reporting and escalation of issues when issues are raised providing clear support to places acting as a key link between the Programme Manager and place based delivery leads.

Present information and issues, explaining highly complex issues, to a wide range of internal and external stakeholders.

To liaise with other Programme Managers nationally to share best practice.

To be the first point of contact for NHS England Personalised Care team;

To provide programme management for multiple projects, working within a multidisciplinary environment, adhering to the relevant standards and methodologies
