WY HCP is going through an
extraordinary period of change, working in partnership with our 5 local places
and across several programmes to support significant transformation. Due to
changing roles and evolving portfolios our team is seeking new, dynamic
leadership to support the evolution of our team with new thinking and ideas. The
area of responsibility is initially Personalisation and Unpaid Carers, but
there is opportunity to work right across the Long Term Conditions and
Personalisation programmes as we continue to connect pathways and systems.

Main duties of the job
Contributing to the strategic planning of the function,
identifying interdependencies across component programmes/projects, potential
impacts on wider organisation, resource requirements and building in
contingency and adjustments, as necessary
Contributing to the financial management of the
function and its constituent programmes ensuring value for money
Ensuring timely and accurate information analysis and
reporting to the Associate Director
Providing operational and strategic leadership to
support transformation, sharing of best practice and achieving the key
performance indicators aligned to all programmes within the function
Working
closely with the Accountable Officers, Chief Executives, Clinical Leads,
Clinical networks and NHS England providing high quality specialist advice,
expertise and leadership on communications and engagement to the function, its
programmes and the Partnership
Ensuring any risks are understood and where
appropriate mitigated through appropriate actions and strategies where
necessary supported by the ICS.
Promoting a culture of effective joint partnership
working ensuring that there is clear reporting and escalation of issues when
issues are raised providing clear support to places acting as a key link
between the Programme Manager and place based delivery leads
Ensuring the principles of openness, transparency and
candour are observed and upheld in all working practices

The post
holder will apply programme management expertise and best practice in managing a
number of programmes and projects in a disciplined manner.The Senior
Programme Manager will directly report and support the role of the Programme
Director and ensure that programmes of work are well established, coordinated and

development and implementation of agreed monitoring systems in place.

To ensure an appropriate degree of rigour and
discipline in relation to the approach,
structure, progress reporting and management of risk within the programme
that is proportionate to the devolved (to place) approach to delivery.
To work in partnership with CCGs, NHS Providers and
Local Authorities and the wider
partnership to ensure that the approach to delivering the Personalised
Care programme is clearly articulated and that each place is supported
to enable each to ensure the delivery of the key objectives within both
programmes and that any risks are understood and where appropriate mitigated
through appropriate actions and strategies where necessary supported
by the ICS.
