The LTCs and Personalisation Function at the award winning West Yorkshire Health and Care Partnership (WY HCP) are seeking to expand the programme management team.

The role will work across West Yorkshire focusing on;

· Embedding a vision to embed personalised care across a range of LTC areas e.g. stroke, diabetes and palliative and end of life care

· Reducing inequalities for people living with a LTC and their carers through the delivery of the Long Term Plan ambitions for personalised care

· Improving recognition, identification and support for unpaid carers and working carers across the West Yorkshire health and care system

· Engage with people and professionals to understand good practice and barriers to personalised care and the impact on the quality and experience of care

As Programme Manager you will be responsible for developing and delivering multiple projects within the programme to time, cost and quality criteria and meeting a range of stakeholder requirements.

Main duties of the job

· Provide excellent programme management of a broad range of complex activities within multiple projects across a range of partners and stakeholders

· Develop and maintain a mixed portfolio of core project documentation ensuring reporting and appropriate escalation of key risks and issues

· Set up and maintain progress meetings at the required frequency

· Manage own workload and make informed decisions independently

· Ensure effective communication with a range of organisations to ensure successful delivery of the programme including managing stakeholder expectations

· Use influencing and negotiation skills and strategies to ensure delivery from project leads for whom there is no direct line management responsibility

· Work closely with stakeholders across West Yorkshire to ensure engagement and buy in

· Support embedding of patient and public engagement into programme

· Support a learning culture through the dissemination of learning outcomes

· Share complex information in appropriate formats to enable decision making

· Co-ordinate the implementation of evaluation plans

· Propose changes to local policies and procedures based on national policy

· Contribute to NHS England’s reporting process

  • Experience and knowledge of commissioning
  • Must be proficient in use of Excel with experience of other software packages.
  • Proficient knowledge of data protection and confidentiality, governance and dealing with extremely sensitive data on a daily basis.
