Job summary

The Reducing Inequalities Alliance is leading a system wide approach to reducing inequalities across the Bradford and Craven Health and Care Partnership (formerly Bradford and Craven CCG) and its partners.

The Reducing Inequalities Lead will support the newly formed Reducing Inequalities Alliance (RIA) and be part of the RIA core team. They will lead strategic programmes to reduce inequalities across Bradford and Craven in support of the Bradford and Craven Partnership’s overall ambition to reduce inequalities.

Main duties of the job

The three central aims of the RIA will be to work with services, communities, & civic institutions to reduce inequalities. The alliance will provide added value to partners by: Setting a strategic vision for reducing inequalities Supporting best practice to reduce inequalities Build capacity around leadership This postholder will work closely with the Associate Director for RIA & its core team in supporting objectives. They will work with partners at community level to share learning on measures to reduce inequalities & facilitate discussion & learning to develop local inequalities plans. This role is a short-term contract or secondment opportunity for 12 months. The post holder will contribute to the following core programmes of the RIA: Present & promote the Bradford & Craven framework for reducing inequalities. Lead & support forums with localities, community partnerships & primary care networks to better understand inequalities in health & care delivery. Co-create with community partnerships outcomes & measures of partnership success in reducing inequalities. Support the collection, analysis & presentation of community based narratives & case studies that describe inequalities, & evidence based measures to reduce them. Develop & promote common tools & approaches for working with RIA partners & Act as One to support their work & priority setting to reduce inequalities.

• Co-create with community partnerships the development of outcomes and measures of partnership success in closing inequalities (and tools to support prioritisation).

• Support the collection, analysis and presentation of community based narratives and case studies that describe inequalities, and evidence based measures to reduce them.

• Develop and promote common tools for working with community partnerships, transformation programmes and strategic board in assessing and assuring their work and

priorities to reduce inequalities.

In addition, the postholder will be required to:

▪ work with the PCNs, Community Partnerships and GP Practices to support a population health management approach to delivering transformational service change and improvement across the whole care spectrum

▪ line manage 2 X Band 7 Reducing Inequalities Managers.

▪ Support the clinical and managerial leadership of the Bradford and Craven Health and Care Partnership (formerly CCG).

▪ Support the Bradford and Craven Health and Care Partnership’s approaches to strategic and annual planning, clinical engagement to inform and refine the strategic plans and approach to working with partners and key stakeholders to formulate Bradford and Craven Health and Care Partnership s strategies.

▪ Provide Bradford and Craven Health and Care Partnership with advice during the development, commissioning, procurement, contracting and performance management of services.

▪ Provide leadership to the Bradford and Craven Health and Care Partnership’s strategic approach to the reduction of inequalities.

▪ Work with colleagues to improve quality and outcomes across whole pathways (self-care, prevention, health promotion, care treatment and rehabilitation) and the full range of service providers; (primary, secondary, community, social care, voluntary and independent sectors) and implement care pathways that are informed by patients and shaped by the principle of improving the patient experience.

▪ Develop evidence-based solutions to the design of care delivery, prevention, and early intervention programmes (developing self-care strategies, models, pathways, and implementation plans); and

▪ Set the direction, establish, and manage projects, co-ordinate activity and ensure appropriate evaluation mechanisms are in place so outcomes can be identified and achieved.

The post holder will also work with the Planning, Performance and Business Intelligence Team in the use of a Population Health Management and community data to deliver transformational service change and support community health and well-being.

Specifically, the post holder will:

• Support the collection and analysis of qualitative and quantitative information to inform reducing inequalities in communities.

• Share the results of analysis in accessible and relevant formats.

• Identify and share good practice.

• Support programmed leads in ensuring that a focus on reducing inequalities is embedded in our local place’s priorities and programmes of work.
