Job summary

As a registered nurse experienced in CHC, you will join a busy team of nurses who play a pivotal role in the CHC process.

Knowledge and understanding of the current national and local policy/Framework Guidance relating to NHS Continuing Healthcare and NHS Funded Nursing Care is essential.

Main duties of the job

Under the guidance and support of the Clinical Lead, the Nurse Assessor will assess individuals for NHS Continuing Healthcare, in line with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing care, revised July 2022.

  • The postholder will undertake statutory reviews and ensure that client's needs have been met and progressed through the appropriate care pathway within agreed timeframes.
  • To ensure all reviews, reassessments include appropriate reports and evidence to support the level of care need identified within the Health Needs Checklist / Decision Support Tool
  • To work with provider services to assist in the development of competent assessors to undertake assessments and reviews.Responsible for collating evidence and information for the Decision Support Tool and liaising with the multidisciplinary team in order for a team recommendation to be made for eligibility for Continuing Healthcare funding.
  • Present information for panel deliberation.To coordinate and facilitate 'fast track' patients in line with operational policy and within agreed timescales. Ensuring the patients that are eligible receives a package of care or placement appropriate to their needs ensuring best value.
