Job summary
The Integrated Care Board (ICB) Community Pharmacy Integration Project Manager will work as part of the Pharmacy and Medicines Optimisation team, and be partially embedded with secondary care team delivering an effective service supporting colleagues across the primary/ secondary care interface and engaging effectively with a wide range of stakeholders through:
- Monitoring, reporting and evaluating on the relevant ICS based delivery programmes, including reporting on progress, risks and issues.
- Managing projects providing direction to ensure delivery is on time, within budget and to the desired quality.
- Provide project and change management expertise to the planning, execution and implementation of projects and programmes.
- Manage key relationships.
- Create and oversee development of project documentation including business cases, project initiation documents, stakeholder engagement mapping, programme plans and Gant charts.
The role will promote an integrated approach to implementation and improvement of referral pathways into community pharmacy services (e.g. NHS Discharge Medicines Service). As such the post-holder will be required to lead on the mobilisation and the local implementation of a national programme of work that will support delivery of the NHS Long Term Plan.
Main duties of the job
ICB Pharmacy Integration Secondary Care Project Officer will work closely with the ICB Community Pharmacy Clinical Lead, engaging with all partners, in line with the ICB 5-year plan and Integrated Care Partnership (ICP) strategy, to deliver the pharmacy and medicines optimisation shared system-wide strategy and operational plan. Linking with national policy, regional and local priorities to deliver maximum value and outcomes for patients, the local population and the NHS.
The Community Pharmacy Integration Project Manager will be directly responsible in supporting delivery of the key ICS strategic programmes to integrate care across primary and secondary care interfaces to general practice and the wider primary care providers across the Hertfordshire and West Essex integrated care system.
The post holder will handle the day-to-day tasks of a project and monitor the big picture. As they manage teams and tasks, they should also track the status of project milestones to ensure everything is on schedule.
The post holder will work with key stakeholders to support the delivery of the four core purposes of Integrated Care Systems (ICSs):
o improving outcomes in population health and health careo tackling inequalities in outcomes, experience and accesso enhancing productivity and value for moneyo helping the NHS to support broader social and economic development.
