Are you an experienced registered nurse (Adult, Mental Health or Learning Disability), or registered Social Worker / Allied Health professional who is motivated by providing outstanding person-centred support and would like to work in a supportive and inspiring environment?

If you’ve answered ‘yes’,then we would like to hear from you. This is an exciting opportunity to develop and utilise your mental health knowledge and skills, to inform the assessment and delivery of the mental health care provided to individuals across Dorset whilst seeking to continuously improve the service offered.

Personal Health Commissioning (PHC) has undergone a full-service re-structure and is continuing a journey of ongoing improvement to ensure we are at the forefront of providing high quality, efficient and sustainable services for our population.

We currently have an opportunity for an individual to join our small, dedicated team to support in the monitoring and review of specific packages of care commissioned to meet the mental health needs of individuals, who are eligible for S117 aftercare, or who require a period of rehabilitation as an inpatient, whilst detained under the Mental Health Act.

Main duties of the job

The role requires an ability to demonstrate professional knowledge, clinical skills and judgement, an analytical and inquisitive mind, and an ability to multi-task in a structured manner. The successful individual will have a strong understanding of mental Health and social care legislation.

The successful individual will need to support and monitor commissioned packages of care, ensuring that the patient is central to all decisions and that the care pathway is commissioned and delivered in a safe, high quality, well led, and supportive environment.

The successful candidate must be able to work collaboratively with Health and Social Care practitioners, patients, families and carers, service providers, and regulatory bodies and always ensure that a high quality and timely service is delivered. The practitioner must demonstrate an openness and honesty and have strong assessment and reporting skills.

Experience of line management is desirable but not essential. Due to the nature of the role, there is a requirement to travel.

NHS Dorset is a welcoming NHS employer and we offer a wide range of employee benefits. We have vacancies for motivated and experienced individuals like you who wish to work as Senior Practitioners across both the Dorchester and Poole sites. This is a great opportunity to join the NHS Dorset as part of the Personal Health Commissioning team as it continues its service transformation and development.
