Health Commissioning (PHC) has undergone a full-service re-structure and is continuing
a journey of ongoing improvement to ensure we are at the forefront of providing
high quality, efficient and sustainable services for our population.
We
currently have an opportunity for an individual to join our small, dedicated team
to support in the monitoring and review of specific packages of care
commissioned to meet the mental health needs of individuals, who are eligible
for S117 aftercare, or who require a period of rehabilitation as an inpatient,
whilst detained under the Mental Health Act.
Main duties of the job
The role requires an ability to demonstrate professional knowledge, clinical skills and judgement, an analytical and inquisitive mind, and an ability to multi-task in a structured manner. The successful individual will have a strong understanding of mental Health and social care legislation.
The
successful individual will need to support and monitor commissioned packages of
care, ensuring that the patient is central to all decisions and that the care pathway
is commissioned and delivered in a safe, high quality, well led, and supportive
environment.
The
successful candidate must be able to work collaboratively with Health and
Social Care practitioners, patients, families and carers, service providers, and
regulatory bodies and always ensure that a high quality and timely service is
delivered. The practitioner must demonstrate an openness and honesty and have
strong assessment and reporting skills.
Experience
of line management is desirable but not essential. Due to the nature of the
role, there is a requirement to travel.

JOB PURPOSE
The post holder will
play a key role in supporting the Personal Health Commissioning Service.
Working within the remit of the National Frameworks for Continuing Healthcare,
the post holder will deliver an effective and competent level of service and
consistently deliver a client-focused service.
The post holder will be responsible
for reviewing patients, ensuring The National Framework for NHS Personal Health
Commissioning and Funded-Nursing Care is implemented and complied with,
according to the principles and processes. As a Senior Practitioner, this would
include a caseload of complex cases and management of Continuing Healthcare
packages of care, ensuring value for money placements which meet the needs of
the individual and identify opportunities for development and improvement to
assist delivery of best practice healthcare.
The post
holder should hold a full UK driving licence and have access to a vehicle to be
able to sulfil the requirements of this post.
POST SPECIFIC, TASKS AND OBJECTIVES
- Working within a team of Personal Health Commissioning Practitioners,
taking a lead role in complex cases, ensuring a high-quality service is
delivered, supporting and mentoring and supporting staff to achieve continuous
