As part of NHS Dorset Personal Health Commissioning Team (PHC),
you will manage a caseload of CHC eligible individuals in accordance with
National Framework for Continuing Healthcare, Undertake eligibility reviews and
assess individuals needs in order to identify most appropriate care options.
You will assist NHS Dorset commissioners in searching for care and make
recommendations on appropriate placements.

Main duties of the job
You will manage a caseload of CHC eligible individuals in
accordance with National Framework for Continuing Healthcare, collaborating
with a range of healthcare professionally, community and acute services.
You must be able to work flexibly and autonomously and work
collaboratively with Health and Social Care practitioners, patients, families,
and carers, ensuring high quality assessments and timely service is delivered.
You must demonstrate an openness and honesty and have strong assessment and
communication skills and a knowledge of the National Frameworks for Continuing
Healthcare, the Mental Capacity Act and Mental Health Act and Care Act
2014.
PHC has 2 offices. East Team is based in Poole, covering
Bournemouth, Christchurch, Poole area, and the West Team is based in
Dorchester, covering Dorset area. This job advert is recruiting to positions
within the East locality and based in Wallisdown, Poole, however you may be
required to provide support across both sites as necessary. There will also be
a requirement to make off-site visits to undertake assessments and attend
meetings and it is therefore essential that you have a full driving licence with
access to a vehicle for business use.
NHS Dorset operates a Monday-Friday 9-5 service.

The post holder will play a key role in supporting the Personal Health Commissioning
Service. Working within the remit of the National Frameworks for Continuing Healthcare, the
post holder will deliver an effective and competent level of service and consistently deliver a
client-focused service.
The post holder will be required to manage a caseload of CHC Eligible individuals ensuring
The National Framework for NHS Continuing Healthcare and Funded-Nursing Care is
implemented and complied with, according to the principles and processes.
The post holder will oversee a caseload of complex cases, undertake annual eligibility
reviews and manage Continuing Healthcare packages of care, ensuring value for money
placements which meet the needs of the individual and identify opportunities for
development and improvement to assist delivery of best practice healthcare.
The post holder will assist NHS Dorset commissioners in searching for care and make
recommendations on most appropriate placement.
This is not a patient facing role, and is predominantly desk based, requiring high levels of
concentration and an ability to analyse health, social care, and other agency records to
identify whether the individual has/had a Primary Health Care Need, in accordance with the
National Framework for Continuing Health Care.
NHS Dorset currently offers a hybrid model of working with a percentage of contracted hours
being on site and the remainder being home based.
The post holder should hold a full UK driving licence and have access to a vehicle to be able
to fulfil the requirements of this post.
