The Personal Health Commissioning (PHC) department within NHS Dorset currently have an exciting opportunity for a motivated and experienced practitioner to join our team as Senior Practitioner. The post holder will be responsible for supporting individuals who have been determined as eligible for CHC under The National Framework for NHS Continuing Health Care, including undertaking yearly reviews.

As a Senior Practitioner within our Ongoing and Review Service, you will hold a caseload of complex cases and be responsible for coordinating packages of care, ensuring value for money services which meet the individual needs. This includes working within the requirements of the Mental Capacity Act and making applications for Community Deprivation of Liberty Orders. The service operates Monday to Friday between the hours of 9-5.

You will excel at building relationships with people based on partnership and respect. As a practitioner you will be person centred and strengths based, committed to ensuring that the voices of the people who use our services are heard and drive the support and services that they receive.

Main duties of the job

The practitioner should demonstrate an awareness of the National Framework for Continuing Healthcare, the Mental Capacity Act and Mental Health Act and Care Act 2014.

We have 2 offices based in Poole and Dorchester and you will be based in Dorchester for this role, however there will also be a requirement to make off-site visits to undertake assessments and attend meetings. It is therefore essential that you have a full driving licence with access to a vehicle for business use. NHS Dorset are currently working to a hybrid model with a combination of home and office working.
