Senior Nurse Assessor

NHS Bedfordshire, Luton and Milton Keynes (BLMK) Integrated Care Board is an ambitious, organisation delivering real improvements in safety, outcomes and experience for its local population.

BLMK ICB are looking for a senior Nurse who is experienced in assessing a variety of children's health needs, is able to demonstrate leadership qualities and proven track record to manage highly complex cases. You will be working as part of a small, friendly, dedicated team who oversee the processes required for BLMK ICB to make recommendations relating to funding packages of care proportionate to need.

The post holder will also work with the team to ensure that BLMK ICB meets national guidance and requirements relating to it's commissioning responsibilities. They will also work closely with the lead provider to ensure that commissioned packages of care are in line with evidence based practice and the National Children's Continuing Care Framework, SEND and Transforming Care

Main duties of the job

This role is responsible for developing, managing, and implementing the processes required for BLMK ICB to make recommendations relating to all funding requests for children and young people (CYP). This currently includes CCC - Physical, Mental health, Learning Disability and Autism as well as Personal Health Budgets.

The post holder will ensure that BLMK ICB meets national guidance and requirements relating to BLMK ICBs commissioning responsibilities, working closely with the lead provider to ensure that commissioned packages of care are in line with evidence-based practice and the National Children's Continuing Care Framework, SEND and Transforming Care.

As a member of the Continuing Care team the role will include working across our four Local Authority areas as well as potential visits to clients that live out side of the region.

The CCC team operates a hybrid working model that supports flexibility in its approach to sustaining a workforce to meet the service needs, this includes home and office working as agreed with the wider CYP Integrated and Personalisation management team

  • Be responsible for the Childrens Continuing Care assessment of children and young people with Mental Health/ LD needs and Physical Health needs. This will involve making judgements about complex facts or situations by analysing, interpreting, and comparing a range of
  • Triage and prioritise referrals and communicate the outcome of the referral with
  • Be involved in the planning, development, and review of care
  • Be involved in Care, Education and Treatment Reviews (CETR) and Local area emergency protocols (LAEPS)
  • Act as a resource for families, and other stakeholders regarding the eligibility and process of application for funding for children includes Childrens Continuing Care, personal Health budgets and other joint packages of care, signposting families, and professionals to alternative services where they are not
  • Build local and reginal networks to ensure expert health advise informs ICB/CCC processes.
  • Review and assess children and young people as per the Childrens Continuing care Guidance and report any changes required within the care package to the multi-agency eligibility and provision panels.
  • Utilise expert skills in assessing a childs condition, including caring for children with complex disabilities, behavioural management and the equipment needed for the child. In conjunction with lead provider and partner agencies e.g., Physio, OT
  • Have strategic oversight of complex discharges from tertiary and acute centres.
