Job summary

Are you a Heath Professional with Continuing Health Care experience or looking to develop skills in Continuing Health Care within the NHS? Are you looking to work within our Integrated Care Board (ICB)?

An exciting opportunity has arisen for professional with knowledge of Continuing Health Care or a desire to develop knowledge of Continuing Health Care. Bedfordshire Luton and Milton Keynes Integrated Care Board (BLMK ICB) are looking for someone who is enthusiastic, compassionate, and innovative, to work with us to take our vision forward, focusing on increasing the efficiency of the ICB Continuing Health Care Team.

The post holder will work as part of an integrated ICB team, alongside system colleagues plus regional and national teams to help deliver the best value from Local and National programmes and services by providing clinical expertise to support Continuing Health Care Team. Our ICB offers superb development opportunities for all staff, and this role is ideal for aspiring practitioners as you will be responsible for leading continuing care across key business areas.

Training will be given for candidates that meet the specifications outlined.

Main duties of the job

To manage a case load of patients through the Continuing Healthcare process, utilising knowledge, and experience to complete holistic assessments and reviews both remotely and in person. Supporting our partners and the public with insight and application of the National Framework.

Additional expectations are to support the existing team with undertaking any CHC related tasks. e.g. Duty, Safeguarding, Care planning, Personal Health Budget referrals, requiring key skills of communication, prioritisation and organisation.

Key Duties and Responsibilities:1. Working within a team of Continuing Healthcare Practitioners ensuring a high-quality service is delivered, peer supporting and mentoring junior staff to achieve continuous improvement2. Managing a caseload of CHC eligible individuals, often with complex needs3. Facilitating and contributing to multi-disciplinary assessments and patient reviews in line with the NHS Continuing Healthcare and NHS-funded Nursing Care National Framework4. Ensuring the principles and duties of Liberty Protection Standards are holistically, consistently and conscientiously applied to the caseload5. Supporting the Continuing Healthcare team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery6. Supporting the Team Leader in ensuring that all processes that are implemented are compliant with the NHS Continuing Healthcare and NHS-funded Nursing Care National Framework7. Commitment to completing all mandatory training8. Deputising as required in the absence of the Team Leader for Continuing Healthcare9. Assisting the Team Leaders for Continuing Healthcare with resolution of complaints, appeals and disputes as well as capturing of lessons learned to mitigate these instances10.
