NHS Somerset ICB is seeking an experienced HCPC registered Occupational Therapist to join the busy All Age Continuing Care/CHC team, which is part of the wider Quality and Nursing directorate.

You will be an expert practitioner in occupational therapy practice and provide clinical expertise, specialist advice for adults, children and young people with complex needs who require assessment and equipment provision within the NHS Continuing Healthcare (CHC) and Children and Young People Continuing Care (CYP CC) criteria.

You will need excellent assessment and interpersonal skills, and be passionate about supporting individuals with long-term complex needs, experience in All Age Continuing Care is desirable but not essential.

You will need excellent organisational skills and a sound, knowledge of MS office packages such as Word, Excel, Outlook, Excel etc., alongside excellent written and verbal communication skills both virtually and in person.

Main duties of the job

  • To act as an expert practitioner for the wider AACC team, other occupational therapy staff, other clinical and social care colleagues in respect of the complex clinical needs of adults, children and young people who meet the CHC/CYP CC criteria.

Undertake assessments for individuals with an identified Occupational Therapy need including adults, children and young people who live in residential nursing homes and specialist placements both in and out of county as well as for individuals in receipt of Personal Health Budgets (PHBs) in their own homes and supported living accommodation across Somerset.

Make decisions on NHS Continuing Healthcare eligibility for adults as part of a Multi-Disciplinary Team (MDT) approach

  • To be able to move and handle adults, children and young people with complex physical problems and disabilities, assess, prescribe and adjust specialist equipment.

To provide expertise to CHC/ CYP CC panels and to the wider AACC team in relation to CHC applications and care provision

To contribute to providing first level contact for disputes and local resolutions between partner agencies/or individuals or legal bodies

To create and maintain effective working relationships at all levels, internal and external to the NHS Somerset ICB, including NHS and non-NHS organisations locally and nationally

To support in the development across the organisation in the personalisation agenda and to support the pathway delivery for Personal Health Budgets within AACC

The nature of the role will include providing and receiving highly complex, sensitive and contentious information, communicating unwelcome financial decisions to members of the public and using interpersonal skills to overcome barriers to acceptance where there may be hostility and antagonism.

Undertake and support audit requirements relating to CHC/CYPCC

To support the development of and implement local policies, systems and processes around CHC/CYP CC, linked to the relevant key national and local priorities

To promote excellent customer care throughout the CHC/CYP CC process ensuring local resolution of any concerns where possible. This will include providing and receiving highly complex, sensitive and contentious information requiring agreement or co-operation where there are barriers to understanding. This relates to external organisations and the public as well as to staff of the ICB and local authority

To support service requirements, undertake training and assessments relating to Deprivation of Liberty Safeguards (DoLS)

Ensure effective timely communication and liaison with key stakeholders, including partner agencies, Local Authorities, GPs, individuals and their families, and care providers

Working with AACC senior managers and cascade information as appropriate

Investigating and responding to any complaints

Provision of expert clinical (within the OT remit) and non-clinical advice in relation to CHC/CYP CC

Working with discharge teams to promote timely discharges and support understanding of CHC/ CYP CC as part of the discharge process

Maintaining expert knowledge of county-wide OT services including public and private sector provision

Undertaking necessary training

Identifying priorities within own workload. Devise and implement programmes to manage workload demands
