Job responsibilities

Purpose of the role

Reviewers are required to carry out reviews into the deaths of people with a learning disability as part of the Learning Disabilities Mortality Review (LeDeR) Programme, for deaths occurring in Somerset.

Reviewers are responsible for undertaking robust and high-quality reviews of the deaths of people with learning disabilities.

This post is hosted by Somerset Clinical Commissioning Group to work as part of the Patient Safety Team within the Quality, Safety and Engagement Directorate.

The post holder will undertake reviews working as part of a wider team and will be expected to support routine care reviews of individuals receiving Funded Nursing Care (FNC) and Continuing Healthcare (CHC) funded care

Key responsibilities of the role

Conduct an initial review of each death based on a discussion with those who knew the deceased person well including families and through reviewing relevant sets of case notes

• Draw learning and recommendations from reviews for the improvement of the care of people with learning disabilities

Provide accurate and timely feedback relating to the progress of reviews or any concerns to the team

To encourage a culture that values openness, honesty, rigour and challenge

Operational requirements

Undertake appropriate training to become a LeDeR reviewer

Receive information about a person who has died for whom a review is required from the local area contact

To update core data collected at notification of the death

Carry out LeDeR reviews which are comprehensive and completed in an accurate and timely manner

Maintain communication with the team as appropriate during the course of the review to update on progress and highlight any problems

• Write an accurate and concise report and submit completed using the on line LeDeR web based platform and methodology

• To contact family members of people with learning disabilities in a sensitive and compassionate manner and involve them in the review as appropriate

Implement best practice, including protecting confidentiality of patient and family information according to Information Governance guidelines and systems

Implement policies and procedures, ensuring the adoption of best practice methodology, standards and thresholds

Manage own workload to ensure reviews are completed within agreed timeframes related to appropriate reimbursement

To demonstrate agility through adapting to the needs of the organisation by working flexibly in response to changing organisational requirements and priorities

Undertake relevant preparation to support routine CHC and FNC reviews as capacity allows

The post holder will be required to act independently taking responsibility for individual reviews, often working alone using their own initiative within the LeDeR terms of reference

Working conditions / effort

The post holder will have to deal with sensitive, complex and in some cases distressing situations in relation to the nature of serious incidents, safeguarding and learning from deaths

The post holder may be required to work unsociable hours to achieve deadlines in some cases

Frequent use of VDUs

The post holder will require good communication and relationship skills because you may encounter hostility when dealing with distressed patients and families.

Person Specification

Experience

Essential

• Knowledge of relevant legislation and guidance relating to the care of people with learning disabilities, including the Mental Capacity Act (2007) and the Equalities Act (2010) (particularly around Reasonable Adjustments)
• Ability to receive, analyse and present complex and often sensitive information
• Experience and understanding of relevant regulatory frameworks and guidance governing
• Knowledge of what good quality looks like in services and/ or learning disability services
• An understanding of the needs of people with learning disabilities and their families
• Confidence to probe further and challenge if necessary
• Able to analyse information objectively and to write accurate and concise evidence based reports
• Good working knowledge of Microsoft Office and able to demonstrate IT skills in order to submit information accurately

Desirable

• A health or social care background
• Experience of working across organisational boundaries within healthcare and wider social care community
• Experience of evaluating evidence and understanding specialist terminology
• Familiar with Serious Case and Safeguarding Adult reviews
• Involving patients and families to inform service improvement
• Knowledge of patient safety and quality improvement

Qualifications

Essential

• Educated to degree level or appropriate health or care qualification and/or significant level of experience within the health, care or advocacy sector
• Evidence of post qualifying and/or continuing professional development

Desirable

• Registered Health or Social Care Professional
• Educated to masters / higher degree or equivalent
• Specialist knowledge and previous experience of safeguarding vulnerable adults and learning disabilities
