Job summary
LeDeR Reviewer Learning from Lives and Deaths of People with Learning Deaths and Autistic People.
An exciting opportunity has arisen for an enthusiastic and driven individual to join the LeDeR team within the ICB as a LeDeR Reviewer. LeDeR is a service improvement programme which aims to reduce premature mortality and reduce health inequalities for people with learning disabilities and autism. This is an excellent opportunity for someone with experience of the health and social care system and a good working knowledge of what quality care should look like for people with learning disabilities and autistic people to join the team to effect change in the local area.
Working with a team of reviewers and supported by a Senior Reviewer and Local Area Contact, the reviewer role is an interesting and diverse post with opportunities to learn from deaths and support change across the system in Somerset as well as linking in with regional and national projects.
This post would suit someone who is a passionate advocate for people with learning disabilities and autistic people, who has an understanding of health inequalities and who enjoys working towards improving outcomes for people.
Main duties of the job
The postholder will be responsible for carrying out LeDeR reviews, working with families and health and social care professionals to learn from the deaths of people with learning disabilities and autistic people. The reviewer will gather information from a range of clinical record systems, as well as from family members, carers, health and social care professionals and others in an individuals circle of support. The reviewer will then use this information to draw conclusions, highlight good practice and areas for improvement, which will be shared with key stakeholders across the system.
As well as completing LeDeR reviews the post holder will have the opportunity to support exciting service improvement programmes across the Somerset system, improving outcomes for people with a learning disability and autistic people.
