appropriately skilled and experienced Registered Nurse/Social Worker/Allied
Health Professional to join NHS Somersets All Age Continuing Healthcare (CHC) team
as a Team Leader.
The post holder will contribute clinical expertise
to the NHS continuing healthcare (CHC) and Funded Nursing Care (FNC) process.
They will lead a team to deliver multi-disciplinary assessments and reviews in
a nursing home or community setting for patients both within in Somerset and out
of county in line with standard operating policy and procedures.
The vacancy is for 37.5 hours a week but part time
hours can be considered.

Main duties of the job
- To manage the day to
day service requirement of CHC, FNC, and Personal Health Budgets (PHBs).
- Lead and manage CHC
Assessors, including supervision, appraisal, and performance review.
- Coordinate reviews for
existing CHC patients and ensure consistency in applying CHC eligibility
policies.
- Lead CHC assessments
and reviews, including out-of-county cases and complex situations requiring
expert management.
- Plan and coordinate
complex care packages and Personal Health Budgets.
- Promote compliance with
the 28-day timeframe from checklist to decision.
- Build effective
relationships across NHS and non-NHS organisations at local and national
levels.
- Support organisational
development in personal health budget pathways and the personalization agenda.
- Oversee individual care
contracts, ensuring quality and safety in partnership with commissioning
managers.
- Ensure excellent
customer care throughout the CHC process, resolving concerns locally where
possible.
- Prepare appeal cases
and liaise with families in sensitive or contentious situations.
