Lead the development and delivery of the LDAN strategy for all age groups, ensuring it meets the needs of the Somerset population.

Responsible for the LDAN programme of work, including Care Education and Treatment Reviews, Oversight Visits and commissioning for individuals

Oversee the transformation of care pathways within LDAN, driving improvements in service delivery and outcomes.

PURPOSE OF THE ROLE:

The Transformation Manager of Learning Disabilities, Autism and Neurodiversity (LDAN) will work

closely with the Associate Director of Mental Health, Learning Disabilities and Autism Commissioning,

plans are developed, maintained and delivered, in line with organisational governance arrangements,

to support effective service redesign, commissioning and performance management in partnership with

system colleagues.

The Transformation Manager will also support the Associate Director in work relating to their assigned

locality, specifically around place-based commissioning at a neighbourhood level.

The Transformation Manager will support collaborative working across organisational boundaries by

building strong and trusting relationships with key system partners to improve outcomes for the

population of Somerset.
