Job summary

NHS Somerset Integrated care board (ICB) have a vacancy for an experienced pharmacist to join us as a Band 8b Locality Medicines Management Pharmacist. Specifically, you will:

provide expert advice to clinicians within a locality (primary care network/s) of 10-15 GP practices on Medicines Optimisation, including policies and procedures to achieve high quality, safe, effective prescribing, and value for money

facilitate medicines optimisation between patients, prescribers, community pharmacies and secondary care, including providing advice on the Community Pharmacy Contract and commissioning of pharmacy services

assist the Chief Pharmacist in advising Somerset ICB and wider Somerset System on Medicines Optimisation issues including health service developments, governance, professional issues, formulary development, introduction of new drugs, patient complaints, pharmacy services.

Main duties of the job

You will be efficient, organised and a good team worker; but also, be able to act as an autonomous practitioner while also being innovative and resourceful. It is an exciting role that will require interaction with a variety of people form senior clinicians to patients.

You will also be expected to lead a team of staff in the delivery of high quality clinical and directorate related pharmacy services. This role includes taking the medicines optimisation lead for Diabetes and Respiratory conditions

KEY RESPONSIBILTIES OF THE ROLE:

Facilitating medicines optimisation between patients, prescribers, community pharmacies and secondary care

Providing advice on the commissioning of pharmacy services

Working with the Associate Director of Medicines Management to advise Somerset Integrate Care Board (ICB) on Medicines Optimisation issues including Health Service Developments, Governance, Professional issues, formulary development, introduction of new drugs, patient complaints, pharmacy services

Overseeing and managing the implementation of specific changes in prescribing medication, formulary changes and medication reviews within General Practice by employed and commissioned staff

Meeting at least annually with each GP practice within their locality and to provide support and influence improvements in the quality, safety and cost effectiveness of medicines optimisation

Providing data to individual practices and federations to assist with peer comparison of medicines optimisation data

Leading on the development and implementation of specific projects as delegated by the Associate Director of Medicines Management

To lead on the cost effectiveness of the commissioned practice support pharmacists and technicians

To provide expert advice to commissioning working groups on areas of medicines involvement in pathways

Providing expert knowledge and input into delegated committees and patient pathway groups e.g. drugs and therapeutics committees

To manage the improvement in the quality, safety and cost effectiveness of prescribing by performance monitoring the implementing specific changes in prescribing medication, formulary changes and medication reviews within the locality group of General Practices

To work closely with the practice manager and prescribing lead of each practice including where possible regular discussions with individual practices within the federations about the annual QOF and Local Enhanced Service targets. Monitor and work with the practices to ensure the targets are achieved

To support the implementation of the ICB medicines optimisation strategy to deliver better clinical outcomes for patients and lead on specific areas as delegated by the Associate Director of Medicines Management

To facilitate the training and improvement of practice staff and support staff, including leading on specific training sessions, contributing to the newsletter and presenting at prescribing leads conferences.

To ensure that CMO/Medicines and Healthcare Products Regulatory Agency (MHRA) and other Drug or safety alerts are reviewed within the practices and federations

To ensure all support staff communicate effectively and successful with patients on medicines optimisation including changes to their medication including clinical and cost effectiveness benefits

To ensure all support staff communicate effectively and successfully with patients on medicines optimisation including patient concordance and reduction of medicines wastage

To lead on the Medicines Optimisation strategy with regard to support provided to social care and care homes

To support the ICB in the delivery of the overall strategy including the clinical outcomes framework and NICE quality standards
