Job summary
An
opportunity has arisen for a state registered occupational therapist with
experience of frailty and complex care needs to join the BNSSG - Funded Health
Care team in role for 37.5 hrs a week.
This
is an exciting opportunity to develop your assessment and critical thinking
skills in a new field of healthcare crossing physical, mental and learning
disability areas. The role will involve
assessing, reviewing and case managing patients with regards to their health
needs, their welfare and provision of care, whether in a domestic setting or
residential placement. You will provide
specialist assessment and advice in the use of assistive technology to ensure
care delivery is patient centred, clinically effective and least restrictive in
approach.
Main duties of the job
Our
team is based in the heart of Bristol covering the BNSSG demographic across
South Gloucestershire, Bristol and North Somerset. We follow
a blended working model of both patient facing and remote working as needed and
continue to deliver person centred assessments of need and subsequent provision
of health care if found eligible under the National Framework for Continuing
Healthcare.
The
job holder will be involved in care coordination as well as highlighting
service improvement and governance issues.
An ability to work collaboratively with other agencies is essential and
experience of working in such a manner is desirable. As this role will involve
working with clients across multiple areas of care, a broad range of knowledge
and experience is welcomed and encouraged.
However, training will be tailored to your experience, the role and the
needs of the service.

- Educated to degree level in BSc or equivalent in Occupational Therapy HCPC Registered or equivalent level of experience of working at a similar level in specialist area
